On 09/22/2021, I completed an on-site inspection. I interviewed program director Jesy Parrish and child care staff members Danielle Lopez and Tamie Korb. Child care staff member Margo Hoagg was not present. Ms. Parrish provided me with attendance records for staff and children and child information cards for Child A and Child B. Ms. Parrish stated that child care staff member Tamie Korb and Margo Hoagg were on the playground at the time of the incidents, but neither witnessed Child A or Child B urinating. One was sweeping the porch while the other was logging information into the Brightwheel app. Children who witnessed 2 Child A and Child B urinate in the back of the playground near the swings reported the incidents to child care staff member Margo Hoagg. No child care staff members were in the back of the playground when Child A and Child B urinated. According to the children who reported the incidents, Child A and Child B did not urinate at the same time. Child A and Child B also stated they urinated at separate times. Ms. Parrish stated that although staff have been trained on active supervision, Ms. Korb and Ms. Hoagg did not clearly communicate with one another and failed to provide adequate supervision on the playground. Ms. Korb stated she has no knowledge of the incidents. The children who witnessed the incidents reported what they saw to Ms. Hoagg. Ms. Korb also stated that Child A's father has let Child A urinate outside in the past, but she has never observed Child A urinate outside while at the center. Ms. Lopez stated she was inside when Child A and Child B urinated on the playground.

On 09/17/2021, child care staff member Tamie Korb provided appropriate care and supervision of Child A and Child B when she observed Child A holding Child B's hand in front of his genital region, moving her hand in a circular motion. Ms. Korb immediately separated Child A and Child B.
