While the class ate lunch, Child A was in the bathroom stall for at least 45 minutes until lunch was over. On 02/17/2021, 1 conducted an on-site inspection. Program director Heather Rosa was not present. Center coordinator Sarah Guffey called licensee designee Brigid Wilson. I interviewed Ms. Wilson by telephone. Ms. Wilson E stated that on 02/09/2021 Child A's Mother reported to Ms. Rosa that Child A sustained cuts to wrist while in care on 02/08/2021. Ms. Wilson stated that CCSM 1 was the on y staff member in Child A's classroom on 02/08/2021. On 02/09/2021, CCSM 1 gave the center a two-week notice to end her employment at the center. CCSM 1 accepted employment at a center closer to her home. On 02/0912021, CCSM 1 reported not feeling well and went home early. Although the center makes it a practice to informs parents when a staff member leaves employment, CCSM 1 took it upon herself to send the parents a "goodbye" letter on her own via text message. A couple of parents responded to let CCSM I know they were sorry she was leaving the center. One parent went on to say she understands how stressful things have been for her, which prompted CCSM 1 to "go on her. offer CCSM 1 began to harass parents with inappropriate text messages. On the evening of 02/09/2021, Ms. Wilson called CCSM 1 to tell her to stop contacting child care parents. Ms. Wilson and CCSM 1 mutually agreed to- CCSM 1's 0 On 02123/2021, Child A's Mother stated that Child A told father that CCSM 1 cut with scissors when he picked ■ up from the center a een 4:30pm and Opm on 02/08/2021. Child A immediately showed her two cut marks on wrist when she arrived home from work at 8:30pm on 02/08/2021. Child A told her that CCSM 1 cut with scissors. Child A was cutting paper when CCSM 1 got mad at 11 and threw paper away. Child A stated, "Mommy, I wasn't paying attention," 11d A had two small linear cut marks on ■ wrist. The cuts were deep enough to scab over and peel off. Child A does not have a wild imagination. Child A's account of what happened has remained consistent. Child A's Mother has been very emotional since the incident. She has had nightmares about her being harmed. She feels helpless that her was injured, andss he was unable to protect ■. Child A's Mother stated that on 02/09/2021, she Mother stopped in Ms. Rosa's office around 12:30pm when she picked up Child A on 02109/2021. She informed Ms. Rosa of the cut marks on Child A's wrist. She showed Ms. Rosa Child A's wrist. Child A told Ms. Rosa that CCSM 1 cut■ with scissors. Ms. Rosa informed her 6 (Child A's Mother) that CCSM 1 went home early and that she also put in her two- week notice. Ms. Rosa advised her to reach out to CCSM 1 to discuss the matter. Child A's Mother was disappointed in Ms. Rosa's lack of urgency regarding the cut marks on Child A's wrist. Child A's Mother left Ms. Rosa's office feeling very unsettled. CCSM 1 had been texting her, so she decided to text her back. She asked CCSM 1 if Child A had an accident with scissors. CCSM 1 denied that Child A had an accident with scissors. She stated children did not have access to scissors on 02/08/2021. Child A's Mother told CCSM 1 that Child A reported that she (CCSM 1) cut wrist with scissors. CCSM 1 denied the allegation. CCSM 1 stated she alrea y reported Child A's injury to Ms. Rosa. On 02/25/2021, I conducted an on-site inspection with We interviewed CCSM 2, Ms. Rosa, CCSM 3, an "Mosmostly saw CCSM 1 with children during outdoor play and when their classrooms combined at the end of the day. CCSM 2 has no concerns with CCSM 1's interactions with children, but she has heard CCSM 1 raise her voice towards a child a few times. CCSM 1 typically talked with the children when there was conflict and provided comfort when needed. Ms. Rosa stated CCSM 1 was hired as a lead caregiver in . CCSM 1's new hire training was interrupted by the COVID shutdown in March. The center resumed child care in August. CCSM 1 is currently in the process of finishing her new hire training. CCSM 1 was particularly hard on herself when she forgot to send othin s home with the

She told Ms. Rosa that Child A reported to her and her husband on 02/08/2021 that CCSM 1 cut with a pair of scissors. Child A's Mother showed Ms. Rosa t e cut marks on Child A's wrist. Ms. Rosa advised Child A's Mother to contact CCSM 1 to discuss the matter. Ms. Rosa did not report the incident to CPS as required by Michigan law.
