The child safety latch that was installed on the inside of the cabinet failed to prevent Child A from accessing the hazardous chemical. Child A ingested some of the chemical from a dishwasher pod and vomited.

Child A gained access to dishwasher pods under the kitchen sink without Michelle Brefka noticing. Child A ingested some of the detergent and vomited.
