Staff did not provide appropriate care and supervision of children at all times. On 10/24/2025, Child A (5-year-old) was left unattended on the playground for 14 minutes after teachers and a group of children went inside. Teachers did not notice that Child A was missing from the group. Teacher 1 saw Child A alone on the playground as she was leaving for the day, and she immediately brought him back to his group.

R 400.8152(4) · R 400.8152(4) Medication; administrative procedures. Prescription medication must have the pharmacy label indicating the physician's name, child's first and last name, instructions, name and strength of the medication, and must be given according to those instructions.

A child with an inhaler did not have the pharmacy label attached.

At the time of a child's initial attendance, a center shall obtain a child information card, using a form provided by the department or a comparable substitute, that is completed and signed by the child's parent. The center shall keep it on file and accessible in the center.

Multiple school age children’s child information cards were missing information in the allergies/special needs/instructions section.
