Staff (S) 1, was not fingerprinted and background checked prior to working at the center. S 1 began working as a cook on 11/10/2025. He was fingerprinted on 11/12/2025 and found "eligible" on 11/13/2025. S 1 meets the definition of a child care staff member because he is employed by the center. He is not responsible for the care and supervision of children, nor is he unsupervised around children in care.

Licensee Designee Marisa Korn and the board of directors did not ensure that the Substitute Program Administrator met the qualifications of a Lead Teacher. On 10/20/2025, I informed a member of the board that T 4 did not qualify for the role of Substitute Program Administrator because she lacked the required trainings. On 11/5/2025, Marisa Korn terminated the employment of the qualified Program Administrator. None of the teachers who were interviewed were able to provide corroborating accounts of instances when Teacher 1 engaged in inappropriate care and supervision of children. None of the parents who were interviewed observed Teacher 1 engage in inappropriate interactions with children.
