The means of egress are maintained in an accessible, free, and unobstructed manner. During the onsite investigation on 1/26/2026, all of the exits to the outside of the building were unobstructed. None of the program staff interviewed reported that they were unable to open their exit doors.

Staff (S) 1, was not fingerprinted and background checked prior to working at the center. S 1 began working as a cook on 11/10/2025. He was fingerprinted on 11/12/2025 and found "eligible" on 11/13/2025. S 1 meets the definition of a child care staff member because he is employed by the center. He is not responsible for the care and supervision of children, nor is he unsupervised around children in care.

Licensee Designee Marisa Korn and the board of directors did not ensure that the Substitute Program Administrator met the qualifications of a Lead Teacher. On 10/20/2025, I informed a member of the board that T 4 did not qualify for the role of Substitute Program Administrator because she lacked the required trainings. On 11/5/2025, Marisa Korn terminated the employment of the qualified Program Administrator.
