The facility did not provide appropriate care and supervision to Child A when he fell from the climbing equipment. On video Teacher was observed on her phone, sitting in a chair that was at least 18 feet from the children climbing on the equipment. She was not properly supervising the children. The climbing equipment was placed over padded mats, but the fall area was not properly covered resulting in Child A getting a skull fracture from his fall.
