On 03/24/2025, Child Care Staff Member 1 was in the classroom with 17 children alone. CCSM 1 did not provide appropriate care and supervision of the children in her care. CCSM 1 was observed sitting in the chair for over 10 minutes with several items blocking her viewpoint of seeing all the children in the classroom. Video footage showed Child A and Child B being alone in the bathroom for over six minutes without staff knowledge of the location of the children.

On 05/15/2021, I contacted Complainant who stated several children were ill with flu like symptoms at the sister facility, Nanny's Infant/Toddler Day Care Center. She believes a sibling brought it to the Nanny's location on Pardee and due to lack of cleanliness, other children became ill. On 05/17/2021, I conducted an unannounced inspection at Nanny's Nursery and Day Care Center. I did not see any obvious signs of it being dirty or unclean. Licensee Ms. Sheryl Westerdale and the program director, Cheryl Neblett were present. They were aware of the situation at the Infant/Toddler location and were on alert due to siblings attending their facility. According to Ms. Neblett, parents were turned away if a child was not out for a full twenty-four hours after becoming symptom free. They closely monitored children and staff for symptoms. Anyone exhibited any signs of illness was sent home and the area thoroughly cleaned and disinfected. The facility is consistently cleaned and disinfected daily. In addition, a Health Inspection was requested to ensure nothing at the facility contributed the illnesses. The director, Ms. Neblett stated a few children had projectile vomiting and were immediately sent home. They were sent home immediately. The facility was(is) constantly cleaned and disinfected. Approximately twelve children exhibited symptoms of the stomach flu, but only three became ill at the facility and were immediately sent home after projectile vomiting or diarrhea. On 04/15/2021, one child became ill who has a sibling at the sister location. He was immediately sent home. On 04/19/2021 through 04/23/2021, a total of nine additional cases of diarrhea or vomiting was reported to the facility by parents. None of the symptomatic children were allowed to attend until free of symptoms for a minimum of 24 hours. All cases were reported to the Wayne County Health Department. On 04/19/2021, Mr. Lezil Iroha an Environmentalist for the Wayne County Health Department conducted an inspection at the facility. He did not find the facility to be "unclean" or detect anything which would have contributed to the children becoming ill. During his inspection, the facility was found to be clean. A copy of his report is on file. On 06/03/2021, I interviewed several parents regarding the allegation. Child A's Mother states Child A became ill while home and she notified the facility. He did not return until fully free of symptoms. She has a child enrolled at the sister facility who also had flu like symptoms. Child A's Mother does not believe the facility is dirty. The staff does a good job keeping things clean and disinfected. It's her belief a "stomach bug" was going around and her children caught it. Child B's Father has no problems or concerns with the center or how the situation 3 was handled. Parents were notified of "a possible stomach bug going around" and asked children to be monitored for symptoms and kept home should any exist. Child C's Mother was immediately notified to pick him up after an incident of diarrhea at the facility. She does not believe the facility had anything to do with his becoming ill. The center is always clean, and she has no problems or concerns with it. Child D's Mother has no problems or concerns with the center. She has always found it clean and is happy with it. Child E's Mother and Child F's Mother has not returned my call as of this entry.

On 11/24/2020, I interviewed the Child A and Child B's Guardian by telephone. She stated Child A was kicked out of the childcare center for kicking a younger child while on the playground and for calling another child "ugly.' She also stated the center was keeping a notebook on Child A and recording his behaviors. She felt Child A was being singled out because they did not keep a notebook on any of the other children. The CCSM did nothing when the other children would interfere with Child A when he was trying to do his schoolwork. The Child A and Child B's Guardian stated Ms Emily was always yelling at Child A. On 11/30/2020, I conducted an unannounced inspection at the childcare center. I interviewed the acting program director Cheryl Neblett. She stated Child A was disenrolled from the center. Child B was not disenrolled and could have continued attending. She and her staff had spoken to Child A's Guardian many times about his behaviors which included him being disruptive to the other children while they were in their virtual class, getting in the face of the CCSM, yelling and swearing at them and refusing to participate in his virtual class. Child A's Guardian had always been receptive to their calls and had come to pick him up when needed. She stated they had been documenting his behaviors and provided copies for me. She stated that on 11/20/2020, Child A kicked a preschooler while on the playground. This incident was captured on video. Child A was disenrolled after this incident. I interviewed CCSM, Emily Lowell. She worked with Child A in the mornings and would help him get set up for his morning zoom meeting. This was Child A's most difficult time as he did not like Zoom and would get under the table and/ or refuse to get on his computer. She denies yelling at Child A. She admits she has a loud voice and can be stern. She stated she would get down on Child A's level and try to talk to him. She stated Child A's Guardian knows he can be a handful and was cooperative with the center in coming to pick him up or talking to him on the phone to get him to calm down. Child A would say he wanted to get kick out so he can stay home. I interviewed CCSM, Skylar Corwin. She works in the school-age classroom. She stated Child A can be quite a handful. He picks on other children. He does not like to do his Zoom meetings and he can be rude to staff and the others. She stated he usually responds to her or her telling him she is going to call his Guardian. She denies yelling at Child A but has had to raise her voice to get his attention. She stated when other CCSM get frustrated with him they will raise their voice. I interviewed Child C (male, age 7), Child D (female, age 9), Child E (male, age 6) and Child F (female, age 7) in a group. They all stated they liked coming to the childcare center. The CCSM are great. They are friendly and help them with their schoolwork. They denied the CCSM yells at them. I asked if they felt safe at the center and they said, yes. I asked if there was any bullying by other children. They stated no, then Child C and Child D began to talk about how much quieter the center is without Child A being present. They stated Child A made "rude comments about them", "he thinks he's funny but he's not," he interrupts them when they are in class. 3 In reading through the notes, Child A was most difficult to manage when it came time for his Zoom meeting with his class in the mornings. There were quite a few documented entries where he was disruptive to the other students in the class and disrespectful to the CCSM to the point where his Guardian had to be called to come and pick him up. ..............
