On 04/14/2021, I contacted Complainant who stated some issues at the facility could be prevented if proper care was taken of the children, food preparation and overall cleanliness. Complainant believes several children became ill after eating lunch and thinks it was food poisoning. On 04/14/2021, I contacted the facility and requested they obtain an immediate health inspection. The director, Kathy Gardner stated a few children had projectile vomiting and were immediately sent home. The day we spoke, she indicated four children were sent home and all were infants who did not eat the food. Ms. Gardner was aware of food poisoning being blamed as the reason, but she had no proof it was true. The food was prepared and transported from a sister facility. No child at that location presented with an illness after consuming the same meal. On 04/13/2021, six children (three toddlers and three infants) were sent home after projectile vomiting. On 04/14/2021, Ms. Gardner indicated four more children (three infants and one toddler) was sent home. To eliminate further exposure, I recommended the facility close for a thorough cleaning and disinfecting. Seeing it was Wednesday, I explained the two-day closure would allow for cleaning and possibly the "bug" to run its course for anyone having been at the facility. I also explained that keeping the center open could lead to additional cases. Ms. Gardner was not in the position to close the facility, but she stated she would call the licensee Ms. Sheryl Westerdale for guidance Ms. Westerdale contacted me to further discuss the issue. She believed families would be angry if she closed due to having just recently reopened after a Covid-19 exposure. She also explained that the facility is thoroughly cleaned daily. Ms. Westerdale stated she would be monitoring the situation and would make a decision regarding closing. 04/21/2021, I conducted an unannounced investigation at the facility and further discussed the issue with the director Ms. Gardner. She indicated the health dept., was out and found no issues relating to food poisoning. She did not believe it was 3 food poisoning as some parents had mentioned. Fish sticks, fruit and vegetables for lunch the day of the initial incident. The food was prepared at another location and transported over to the facility using the method approved by the local health department. No child at the facility where the food was prepared showed any symptoms after eating the identical meal. According to Ms. Gardner, parents informed her of a "stomach bug" going around and several people in the area had become ill who had no connection to Nanny's. On 04/13/2021, she immediately notified families, via the Brightwheel app, of children being sent home from the infant room after projectile vomiting. Parents of children exhibiting signs of the illness received a telephone call. A notice was also posted on the entrance door of the center. There were no other symptoms at the facility. Parents were asked to monitor children for symptoms and to keep their child home should any develop. Per the center's exclusion policy, the center did not allow any child to return until 24 hours free of any symptoms, which angered some parents. All children who became ill were immediately relocated to another area to wait for parents to pick them up. The area impacted by the illness was thoroughly cleaned and disinfected. A total of 19 children became ill the between 04/13/2021 and 04/16/2021 Nanny's Infants and Toddler Center reported the incident to the Wayne County Health Department. A copy of the submitted information is on file with this department. Child A's Mother states Child A became ill and was immediately sent home. The illness happened a few days of reopening after being closed for a COVID-19 exposure. On 03/24/2021, Nanny's Nursery self-reported, "A child was running and crashed into a wall causing an injury that required stitches." On 03/25/2021, I interviewed Child A's Mother and Child A's Father. The facility notified them immediately following Child A's injury. According to Child A's Mother, the facility contacted her regarding the incident, but she does not feel they conveyed how serious it was. She was told Child A fell and had an injury. They also stated, "You might want to take her to the hospital." When asked if she was bleeding, they responded "Yes." Staff reported Child A was running, tripped and hit the wall. After arriving at the facility and seeing the deep laceration above her left eye, she immediately knew it wasn't just "a wall" that caused it. A staff later showed Child A's Father the electrical outlet she hit when she fell. He states, the metal wall outlet is dangerous and should not be in a child care facility. Child A was transported to Beaumont Hospital in Taylor where she received four stitches. On 04/23/2021, I followed up with Child A's Mother and learned the laceration has healed but left a scar. She is doing ok, but no longer attends the facility. I requested and received a photo of the injury. On 03/25/2021, after learning about the electrical outlet, I called the facility and requested barriers be placed around the outlets to prevent further injuries. According to the program director, Ms. Kathy Gardner, they keep the outlets covered. Staff is constantly having to redirect children from the area and putting the barriers back in place since children often move them. On 03/25/2021, I contacted Qualified Fire Inspector (QFI), Gregory Smith. I explained the seriousness of the incident and requested he go out to inspect the placement of the outlet(s). On 03/26/2021, Mr. Smith completed an onsite inspection and requested an electrician lift the units higher so children could not access them. He believes the location of the units was a flaw in the construction. Children are not to use the area until the units are removed. On 03/30/2021, I conducted an unannounced inspection at the facility and further discussed the issue with Ms. Gardner. I also inspected the location and installation of the electrical outlet involved in the incident. It's in the rear of the classroom, made of galvanized metal and sits low; approximately 12 inches off the floor. The unit is not recessed to the wall and extends outward approximately 2-3 inches. I had to move play equipment to access the area. During the investigation, I found the facility was clean, adequately staffed and children were being supervised. Ms. Gardner was not present during the incident, but staff informed her of Child A, and other children, preparing to go outside and they were excited. They started 3 running toward the rear of the building when Child A tripped and hit the wall. Staff ran to help her and a cut was observed bleeding from her forehead. Her parents were contacted immediately. First aid was applied by providing pressure to the lacerated area and a cold compress. Child A was not crying uncontrollably or exhibiting any signs of discomfort. According to Ms. Gardner, the outlets are not used by the center and they typically use barriers such as toys or shelving units to prevent children from playing around them. Ms. Gardner also states the children often move the barriers but typically staff puts them back. She believes staff do a good job supervising the children and they feel "awful" about the incident. On 04/13/2021, I interviewed Child Care Staff Member (CCSM) Marilyn Simpson and Maegan Merrow. Neither of them witnessed anything and had left earlier in the day before the incident occurred. I also contacted CCSM Jasmine Baker, Sarina Lara, Sheryl Ratliff, and Jessica Rueger. They have not returned my call as of this entry.
