On 05/11/2023, center director Monica Mitchell called and reported Child A was outside on the playground and stuck a stick in his nose. Childcare staff members (CCSMs) were not certain when, or if, it happened since nothing was found when they initially examined him. His mother arrived shortly thereafter and was informed of the incident. There were twenty children and five CCSM. The outdoor play area is checked daily, and any hazards are removed. On 05/16/2023, I called and discussed the incident with Child A's Mother. I was unable to reach her before that date. The day of the incident, she arrived at approximately 5:25 p.m., to pick up Child A. She observed him playing without incident on the playground. CCSM 1 informed her of Child A saying he had put a stick in his nose, but no one saw him do it or could locate it when they checked. Upon arriving home, Child A's Mother and Child A's Father examined him and observed a stick far up in his nose. They were unable to remove it and transported him to Beaumont Urgent Care in Taylor. Once there, they were told to take him to Beaumont Emergency where the stick was removed. Child A's Mother reports the stick was far up in his sinus cavity and near his brain. Since the removal of the stick, he was diagnosed with a sinus and ear infection. According to Child A's Mother, his continuing to play at the center was dangerous due to where the stick was lodged. Once home she called the facility and notified them of the emergency room visit and requested an incident report. Child A is doing well but no longer attending the facility. A photo of the object removed from his nose is on file with the department. It appears to be a stick approximately an inch long and thick in circumference. On 05/18/2023, I conducted an unannounced investigation at the facility. I inspected the outdoor play area and interviewed CCSM 1. There were no obvious signs of any hazardous objects in the area. There are trees in the area, but no branches were observed on the ground. CCSM 1 did not see the incident. Child A and "other staff' told her he stuck a stick in his nose. She looked and did not see anything and within ten minutes his mother arrived to pick him up and she informed her of the incident. Child A never exhibited any signs of distress or discomfort. On 05/25/2023, I interviewed CCSM 2. She was monitoring children in the back of the playground when CCSM 3 came over and told her Child A said he stuck a stick in his nose. CCSM 3 did not have on her glasses and did not see anything when she looked but wanted her to check. CCSM 2 and CCSM 3 took Child A in the center and used a flashlight on her cellular (CCSM 2) to check his nose. CCSM 2 saw "something" that looked like a stick, but it was too far up, and she did not think it should be removed by staff. She suggested the parent be called but states "The mother showed up approximately ten minutes after they went back outside. 3 On 06/05/2023, I interviewed CCSM 3, the day of the incident she was monitoring Child A and approximately three other children while on the playground. They were closest to the picnic table and Child A kept picking up pine needles from the ground. She would brush them out his hand and redirect him to play with some of the outdoor play equipment. At one point he crawled under the picnic table and was playing with a stick. She took the stick from him and turned to give one of the other children some attention. Child A was in front of her and kneeled. He stood up and said, "I put a stick in my nose." She checked his nose and saw nothing. CCSM 2 was closest to her, so she asked her to check. They went inside the facility where the lighting was better. CCSM 2 used a flashlight to look in Child A's nose and she saw "something." Soon after they returned outside, Child A's Mother was pulling in the driveway.
Child A (3-year-old, male) stuck a stick up his nose and required medical attention. Once found, childcare staff allowed him to continue playing.

Child A was allowed to walk from the building outdoors while child care staff member, Jody Kipfer watched through the window. Complainant was on the phone with Child A and heard child care staff member, June Bedo yelling at Child A while she was on the phone with her. Complainant stated children are being yelled at and treated poorly by staff. Children are made to wait and eat after other children are fed first. Sometimes they were not fed at all.

On 07/21/2021, licensee Mrs. Monica Mitchell reported Child A had sustained an injury while playing outdoors at the facility. She was not present when the incident occurred, but staff informed her of Child A running, falling, and complaining of pain when they went to help her up. Child A's Mother was called and she transported her to a local hospital. Child A was diagnosed with a fractured clavicle. On 07/23/2021, I discussed the incident with Child A's Mother. She stated, Child A is okay but experiencing severe pain. She was prescribed some "heavy pain medication" for the injury after being transported to Henry Ford in Brownstown. Child A was diagnosed with a fractured clavicle and would be staying home for a while. The medication makes her sleepy and she does not want to risk Child A injuring the area. According to Child A's Mother, the facility called immediately following the incident and reported Child A was experiencing severe pain in her right shoulder. "They didn't appear to know what really happened." She was informed of Child A playing a game of falling or running when she injured herself. She was also told the grass was wet. She's not clear why children would be allowed to play on wet grass and does not know if it contributed to the fall. Child A's Mother has never had any problems or concerns with the facility and believes they provide appropriate care and supervision. Child A has returned to the center with her right arm in a sling to minimize use of the area until fully healed. On 07/27/2021, I conducted an unannounced investigation at the facility and inspected the outdoor play area located in the rear of the building. Children can play within a grassy area or the designated concrete space. I did not observe any hazards in the area. There is no inground equipment. The day of the incident, CCSM, Ms. Jody Kipfer was present. She was outside at approximately 11:00 a.m., with twenty-two children and two additional staff members, Shelby Kalicki and Ruby Fleming. The grass was a little wet from morning dew, but not soaked. Child A repeatedly ran around the grassy area despite being told to stop. She was wearing what Ms. Kipfer believed to be a style of shoes that did not give her adequate support when running. At one point she fell and started crying. Ms. Kipfer went to assist her and found no obvious signs of injury, but Child A complained of pain in her right arm. She helped her into the building, applied an ice pack. and called her mother. 08/04/2021, I interviewed Ms. Shelby Kalicki. She stated, "I saw the incident as it unfolded." Child A and several children were outside playing. The grass was a little 3 wet, but not saturated or hazardous where it would be unsafe for them to be playing on it. Child A was running and would deliberately fall. She was asked to stop on several occasions, but she continued to run. The final time, she was running and fell on her right side and begin crying. Ms. Kalicki could tell by the intensity of the crying Child A was hurt. Ms. Kalicki and Ms. Kipfer immediately went over to provide assistance. There were no obvious signs of an injury, but Child A complained of pain in her right shoulder and arm area. On 08/11/2021, I interviewed CCSM Ruby Fleming. She was present the day of the incident and although outside with Child A, she was busy monitoring another group of children and did not see anything pertaining to the incident. When asked if the grass or ground was wet and presented as hazardous she stated, "I don't think so, but don't remember." 4 According to Mrs. Mitchell and CCSM Ms. Kalicki and Ms. Kipfer, if the grass is wet from heavy rain children are not allowed to play on it.

On 5/18/2021, I interviewed complainant. Complainant stated that she was on the cell phone with Child A (eight-year-old female) when she heard 2 child care staff member, June Bedo yelling at her. She was yelling at her for stepping into the parking lot. Child A was trying to get child care staff member, Jody Kipfer's attention indoors at the window. Child A was sent outside by herself to the outdoor play area, while child care staff member, Ms. Kipfer watched her from the classroom window. Complainant heard Ms. Bedo tell Child A to mind her own business and worry about herself. Child A was trying to give a message to Ms. Kipfer from her mother. Complainant spoke with Ms. Kipfer regarding the incident and was informed that Ms. Bedo apologized for yelling at Child A. Child A informed complainant that Ms. Bedo did not apologize, and she also overheard her say to Ms. Kipfer "so what if I didn't apologize." Complainant stated there were other times when Child A has overheard Ms. Bedo and Ms. Kipfer saying inappropriate things about her. Complainant has reported her concerns to licensee, Monica Mitchell, but nothing has been resolved. On 5/18/2021, I interviewed Ms. Mitchell. Ms. Mitchell stated that she was not present during the incident. She was informed that Ms. Kipfer was inside with children watching through the window and Ms. Bedo was outside waiting to receive Child A. Ms. Mitchell was informed that Ms. Bedo yelled at Child A while she was on the phone with her mother and never apologized. Child A was on the cell phone with her mother and was not watching where she stepped. She is not aware if Ms. Bedo apologized to Child A for yelling at her. On 5/18/2021, I interviewed Ms. Kipfer. She stated that Child A walked out the door by herself while she watched from the window. Ms. Bedo was at the gate waiting for Child A. Child A was on the phone with her mother and ran back towards the building. She stepped off the grass near the handicap parking spot. Ms. Kipfer stated that she ran outdoors and approached Child A. Ms. Bedo ran over to her as well. They later went inside for lunch and nothing was said about Child A, Child B (six- year-old female) and Child C's (four-year-old female) Mother. Ms. Kipfer is not aware if Ms. Bedo apologized to Child A for yelling at her. She stated that Ms. Bedo said that she apologized, but she did not witness the apology. Ms. Kipfer also stated that she has not had any inappropriate conversations regarding any parents or children. On 5/19/2021, I interviewed Ms. Bedo. She stated that she panicked when she saw Child A near the parking lot. Ms. Bedo was walking with another child towards the front of the playground when she saw Child A. She went to Child A and asked what she was doing. She stated that she spoke with Child A's Mother later and explained that she panicked and apologized for raising her voice. 3 ...... ................... On

Complainant stated that Child A, Child B (seven-year-old female), and Child C (five-year-old female) were treated different during meal times. They were made to sit in an area away from the other children near the kitchen. Complainant was told that the younger children needed to be fed first. Complainant stated there were times Child A, Child B, and Child C would not get fed. Complainant provided food for Child A, Child B, and Child C and it was thrown away. Child A, Child B, and Child C were sent to the lower level of the daycare to be cared for by other child care staff members. They were being fed and there were no concerns. There were staff changes and Ms. Bedo and Ms. Kipfer were later moved to the lower level caring for Child A, Child B, and Child C again. Complainant removed the children from the center. Ms. Mitchell stated that all children are fed. She stated that due to Covid she was informed that school age children should not be mixed with the daycare children. Because Child A, Child B, and Child C is one family, Ms. Mitchell kept them together away from the other children. She stated that the children were fed when the other children were fed, the younger children received their meals first. She stated they are fed more than others if there are leftovers. No one has ever been forgotten or mistreated. She stated that Child A, Child B, and Child C were brought in various hours of the day. Breakfast is no longer available if the children are brought in at 10:00 AM. Child A, Child B, and Child C's Mother brought the children in and stated that they have not eaten. Child A, Child B, and Child C's Mother text her about it. Staff cannot stop in the middle of circle time or going outdoors to feed the children. She was also asked to feed the children before dropping them off if they were coming in late. Signs were posted on the door and throughout the center that outside food such as fast food was not allowed in the center. She would continue to bring it in. Ms. Mitchell stated that she was not aware of food being thrown away. 4 She stated that no other parents complained about Ms. Bedo or Ms. Kipfer. She has not heard of them speaking inappropriately around the children. Ms. Kipfer stated that Child A, Child B, and Child C were always fed. She has never deprived any children from eating. She stated that she threw empty cereal boxes away that were in the cubbies. Ms. Kipfer stated that she has not said anything inappropriately regarding Child A, Child B, and Child C's Mother. Ms. Bedo stated that she always made sure the children were fed. Ms. Bedo asked the children to wait for the other children to receive first serving before she could give them seconds. She stated that they were never forgotten or mistreated. Ms. Bedo stated that their cereal was never thrown away. Child A, Child B, and Child C brought cereal during circle time and they were asked to put it in their cubbies until snack time. She stated the children ate the cereal later. Ms. Bedo stated that she never talked about Child A, Child B, and Child C's Mother. She stated that she never made a comment about not apologizing to Child A. She also stated that she and Ms. Kipfer have not said anything inappropriate about the children mother. On 5/24/2021, I interviewed Witness. Witness stated the children were not treated fairly when they were being cared for by Ms. Bedo and Ms. Kipfer. Witness stated that Ms. Bedo and Ms. Kipfer did give food to the children if the children arrived after breakfast was over even though they felt like Child A, Child B, and Child C were a nuisance and could fend for themselves because they were older. They would be given a handful of popcorn and expect the popcorn to hold them over until lunch. Ms. Bedo and Ms. Kipfer were spoken to regarding the way they treated Child A, Child B, and Child C different from the other children. Ms. Kipfer was loud and yelled at the children. On 11/30/2020, I conducted an unannounced onsite inspection and observed caregivers providing care and interviewed the licensee, Monica Mitchell. Mrs. Mitchell denies any CCSM yell or mistreat any of the children and she would not allow it. No parent has ever complained to her about any CCSM. She believes the accusations were made by a previous caregiver who she had to reprimand for inappropriate behavior. On 12/07/2020, I interviewed Child A's Mother, Child B's Mother, Child C's, Mother and Child Ds Mother. Neither of the parents had any knowledge of children being yelled at or anyone being mean to them. None of the parents voiced concerns about the facility. They report to have had a wonderful experience with the center. Child A's Mother and Child B's Mother stated the facility does a great job with the children and she has never received or heard of a complaint regarding any staff person. On 12/08/2020, 12/10/2020 and 12/15/2020, I interviewed CCSM 1, CCSM 2, CCSM 3, CCSM 4 and CCSM 5. They all deny having knowledge of anyone yelling or being mean. CCSM 1 denies she has ever yelled or is mean to a child. She is hurt anyone would accuse her of doing anything other than caring for the children.

On 11/30/2020 and 12/16/2020, I completed unannounced onsite inspections to the facility. During my inspections, I interviewed program director, Monica Mitchell, reviewed staff and children's attendance records and observed each classroom. Mrs. Mitchell reported classrooms always operate within the required staff to child ratios. She schedules sufficient staff members daily, but when someone calls in or is running late everything is thrown off. Ms. Mitchell states the current pandemic has made it difficult to maintain staff, but she does her best to keep the center running properly. She fills in whenever possible or classrooms are merged together to provide for sufficient staffing and to ensure ratios are maintained. Mrs. Mitchell provided attendance records for CCSM and children in care. Based on the attendance records reviewed, staff to child ratios have been maintained. I also monitored every classroom and staff to child ratio guidelines. I observed: The Toddler Room had 4 toddlers and 1 CCSM. The 2 to 3 -year-old Room had 10 children and 2 CCSM. 1/2 The lower level had 10-12 children, 3 to sometimes 7 years of age with two CCSM. During the course of the investigation, I interviewed CCSM's who stated the maintaining of ratio was difficult in the past when too many school age children were in the building and disrupting programming. The facility is licensed for school age children, but CCSM's believe the closing of schools due to the pandemic increased the numbers during the day and created ratio problems. Ratio and overall structure in the center is better due to Mrs. Mitchell lowering the number of school age children allowed in the building. CCSM 1 denies the facility has ever been out of ratio. Due to the current pandemic, parents have not been allowed in the building in several months and therefore could not speak to any current ratio concerns.

Based on the information reviewed and observation of classrooms during unannounced investigations, the lower level has a blended classroom made up of children 3-7 years of age. CCSM's are aware ratio must be maintained based on the age of the youngest child. There was never more than 20 children, with no less than two CCSM. ..............................................
