On May 23, 2022, I received a telephone call from Amanda (Mae) Bendall, to report that Child A was injured during morning latch-key child care and required outside medical treatment. Child A was playing a game in the gymnasium called Mr. Fox, which is similar to Red Light, Green Light. Child A tripped over another student who had fallen in front of him and hit his head on the vent cover. First aid was applied by 2 Cheyenne Tushim, child care staff member. Child A was taken to the office and Child A's Mother was contacted to pick him up. Child A received six staples in his head at urgent care. On May 24, 2022, I interviewed Child A's Mother. Child A received outside medical treatment at urgent care and six staples in his head. Child A received prompt first aid and medical treatment and was well cared for by Cheyenne Tushim during the incident. Child A's Mother is thankful for that care that he received. Child A reported to Child A's Mother that he was playing with Mr. Fox when another student pushed him, which caused him to fall. Child A's Mother does not have any concerns with the care and supervision Child A receives at the facility and stated "accidents happen." On May 24, 2022, I received two photographs from Ms. Bendall showing the vent cover that Child A hit his head on. The second photograph shows the lower left-hand corner of the vent not appropriately secured to the tile wall. The vent cover was sticking out away from the wall. Ms. Bendall reported she submitted a work order to the district immediately following Child A's injury to have the vent cover fixed. Ms. Bendall reported that it was fixed on May 24, 2022, and appropriately secured to the wall. On May 25, 2022, I completed an onsite inspection with Ms. Bendall to confirm that the vent cover was appropriately secured to the wall in the gymnasium. I viewed the vent cover had a new screw in the lower left-hand corner and was no longer sticking out from the wall. Ms. Bendall also purchased thin foam padding to cover the corners of the vent covers. Ms. Bendall is in the process of speaking with the district about considering replacement of the vent covers so that they are recessed into the wall. I provided technical assistance to Ms. Bendall regarding the condition of the vent covers. The covers in the gymnasium had chipping paint in multiple locations. I also provide technical assistance to Ms. Bendall regarding the door from the gymnasium to the outside. Light is visible under the door edge, and the frame is deteriorating on the inside.
