On 12/21/2021, the lead coordinator of the center, Amanda Bendall, left a telephone message regarding an incident at the center involving Child A's whereabouts being unknown for a short amount of time. 2 On December 22, 2021, child care licensing consultant Shelly Naples spoke with Ms. Bendall and preschool coordinator Krissy Small regarding the incident. According to Ms. Small, when she arrived at the center on December 17, 2021, around 4:45pm, Child A was sitting in the gymnasium in the "calm down area" on the stage, following an altercation with Child B, his sibling. Ms. Small had come in from bringing in supplies and child care staff workers Ariel Wallace and Paige Feudale informed her of an incident between Child A and Child B. When she went to the stage area, she noticed that he was gone. Her and Ms. Feudale immediately looked around for him and could not find him. They ended up finding him outside of the building in the parking lot. When she spotted him, he immediately began to run away from them. From that point they followed him and were able to coax him to stop and talk. They then contacted Child A and B's Father regarding the incident. Child A has since returned to the center for care. The center is closed until January 04, 2022, due to the holidays. On December 29, 2022, I interviewed Child A and B's Mother. She was aware of the incident and corroborated much of what was reported. She stated that Child A has suffered from bullying in the school district, and he has anxiety when he gets stressed. He reacts with a flight or fight mode and this is what happened unfortunately after the incident with Child B. Child A had told her that Ms. Feudale was in the side room by the gym dealing with another issue when he was fighting with Child B. She came into the gymnasium to see what they were fighting about and yelled at him, so his anxiety went way up. He acknowledged that he ran away from the staff after that and was hiding. She had no concerns that the staff did not supervise him adequately. They called her husband as soon as it happened and by the time he got there within 2-3 minutes he was already located outside the front of the building. In the past, he has run from the school building (during the school day) before when past incidents have happened. This has not happened since June 2021, so they did not think he was still doing this. She indicated that the center staff was aware of his past issues because when he enrolled him this fall she recalled telling them that he cannot ride the bus due to past bullying issues. The child care program has not implemented any special instructions for supervising Child A because she did not think he needed them. The school and center staff have been great, and she is not concerned at all from their end of it. Since the incident, he has been taking a new medication to help address his feelings and behaviors. She has already seen a big difference. On January 07, 2022, I conducted an unannounced onsite inspection at the center. interviewed Ms. Wallace and Ms. Feudale regarding the incident. Both staff corroborated Ms. Small's statements. Ms. Feudale stated that she and Ms. Wallace were both working that afternoon when it happened around 4:30pm. She had been in the side room of the gym when the altercation happened between Child A and Child B. She came in the gym to address the behaviors and talk to them both. He was upset about the altercation and was not very responsive to their instruction. They did not put him specifically in the stage area to calm down, he had chosen to go over there. After she spoke with him, he kind of wandered around the room and 3 ignored everyone, so they let him wander so as to not further antagonize him. She returned to the side room and Ms. Wallace remained in the gym. Shortly thereafter Ms. Smalls arrived to pick up her child and was coming into the room. They explained what happened and she planned to speak with him. At that point they realized that he
