On 02/01/2023, I completed an unscheduled on-site inspection. I spoke with program director Belinda Gulley. On January 27, 2023 at 9:05AM, Ms. Gulley received a message on her phone to go to the gymnasium. When she arrived at the gymnasium, Angela Pratt, lead caregiver in the Woolly Bear classroom for children ages 1-2 years old, said that Child A, picked up a pill under the blue chair 1/2 in the gymnasium and put it in his mouth. Ms. Pratt saw Child A put something in his mouth and asked him to give it to her. Ms. Pratt gave Ms. Gulley the pill that Child A had put in his mouth. Ms. Pratt then went to make a bottle for Child A. Ms. Gulley texted Witness 1, who is the chaperone of the community group who uses the gymnasium twice a week in the evenings for basketball practice. She asked Witness 1 if anyone may have dropped a pill in the gymnasium. Witness 1 said that he had aipill in his pocket that may have dropped out. He sent a picture of 2 his prescription bottle and a pill to Ms. Gulley. Ms. Gulley contacted Child A's Mother immediately and took Child A to Sparrow emergency room. Child A was diagnosed with and was monitored for four hours then released. The child care center allows other community groups to use the gymnasium during times outside of child care hours. The custodian in the building has a schedule of when she cleans the gymnasium and the last time it was cleaned was January 2, 2023. Since then, the basketball teams are the only groups using the gymnasium besides the child care children. The child care children used the gymnasium on Thursday, January 26, 2023 and there was nothing on the floor. Witness 1's basketball team used the gymnasium on Thursday evening, January 26, 2023. During the evening of January 27, 2023, Witness 1 texted Ms. Gulley and said that he did not drop a pill in the gymnasium, and it was not his. It has not been normal practice for child care staff members to walk through the gymnasium prior to the children playing in there to look for potential hazards. There were three children, and two adults present at the time of the incident. Since the incident, Ms. Gulley, Ms. Simpson and the janitor will alternate completing a morning walk through of the gymnasium before children use it to ensure there are no potential hazards. During the on-site inspection I viewed the gymnasium. It is a large, open room with a stage and two chairs placed in front of the stage. I viewed the video recording of the incident. The video shows child care staff members Ms. Pratt and Julie Miller sitting on the floor of the gymnasium. At 9:27:53AM Child A picks something up off the floor and puts it in his mouth. At 9:28:02AM Ms. Pratt walks over to Child A, and he hands her something. Ms. Pratt looks at it, then shows it to Ms. Miller. Ms. Pratt picks Child A up and leaves the gymnasium. It was approximately 49 seconds between when Child A put the object in his mouth and when Ms. Pratt takes it from him. I interviewed Angela Pratt, child care staff member. She is the lead caregiver in the Woolly Bear room. Ms. Pratt started working at the center in September 2022. She was present the day of the incident. Ms. Pratt and Ms. Miller, assistant child care staff member, took the children to the gymnasium around 9:00AM There were three children and two adults present. One of the children was having a "meltdown," so Ms. Pratt was trying to de-escalate the situation with him on her lap, but she was facing the children. She was texting the parent of the child on her lap to let them know how the child was doing. Ms. Pratt looked up and saw Child A put something in his mouth. She walked over to him and told him to give it to her. Child A handed her a pill. Ms. Pratt showed the pill to Ms. Miller, picked up Child A, and went to find Joy Simpson, the office manager. Ms. Pratt told Ms. Simpson what happened, and Ms. Simpson called Ms. Gulley. Ms. Pratt made Child A some formula and he drank about six ounces. Ms. Pratt was present in

The complainant re orted that Child A (male, age 7 years) tried to hold a child down to while at a friend's home. When Child A was asked about what he did he reported that the same thing happened to him two weeks prior at the center while he was in the equipment room getting a ball inside the gym. Child A stated that the when the incident occurred, he Child A did not report the incident to any child care staff members. Child A also stated that he did not report this incident to his mother. After Child A reported the incident to the complainant and Child A's Mother was informed about the 2 incident. Child A's are by the center where Child A is enrolled. Child A's Mother stated that she She does not work with children other than to while they are supervised by child care staff members. Child A's Mother stated that while Child A was in his father's care Child A and attempted to - Child A's Father called Child A's Mother after the incident and informed her about the incident and reported that Child A told him that a child at the center had done the same thing to him. When Child A's Mother asked Child A who the child was Child A gave a name of a child that is not enrolled at the center. Child A's Mother stated that Child A Since that time Child A has been in until the incident while in the care of his father. Child A's Mother stated that Child A's Father was not aware that Child A was until now. Child A's Mother stated that she does not believe that Child A was - and by a child at the center. She stated that Child A has been telling "lies" a lot lately which he is working on in _. She believes that Child A made up a story to Child A's Father to explain his own behavior. Program director Belinda Gulley stated that she knew nothing about Child A alleging that while his in care until Child A's Mother reported it to her on . Child A gave a name to Child A's Mom for the child responsible for the incident. Ms. Gulley stated that there is no one enrolled in the center with the name that Child A gave. Ms. Gulley stated that Child A reportedly told Child A's Mother that the incident occurred in the equipment room in the gym. Ms. Gulley stated that the door to the equipment room in the gym always remains open. Children ask permission from the child care staff members in the gym to go into the equipment room to get a toy. Children are not in the small room long reported incident to happen. Ms. Gulley stated that tl that work in the school-age room are A's Mother told her what Child A reported about the incident, she spoke with stated that the children are not in the equipment room long enough for anything like what Child A reported to have happened. Ms. Gulley stated that she has never had any type of incident like this happen at the center. Child A would have told her if someone at the center was bothering him. Child care staff member stated that Child A has never reported to her that he was held down or by anyone at the center. - stated that Child A is "pretty aggressive" so if something like that happened there would have been a fight. Child A is good at expressing himself when he's upset. ■ = stated that children are never left alone at any time. - stated that when she works with children in the gym, she doesn't allow them in the equipment room in the gym. Children ask for what they want and either she or go into the gym to get the equipment. Child care staff member stated that Child A never told him that by a child at the center or that stated that when children are in the gym, they are not allowed to enter the equipment room. The
