CCSM 1 failed to provide appropriate care and supervision to Child A when she called Child A over to push them while saying “let me show you what you did to him”. CCSM 1 grabbed Child A’s shirt in the chest area and pushed Child A on the ground. CCSM 1 then helped Child A up after he started to cry and apologized to Child A stating “I didn’t know you would fall down like that”. This was witnessed by CCSM 2. During interviews with Child A’s Mother, CCMS 3, Witness 1, Child D’s Mother and Child F’s Mother and Father, all reported incidents where CCSM 1 failed to provide appropriate care and supervision to child care children. Child A's Mother stated Child A casually brought up being pushed by CCSM 1 to her partner recently during dinner. CCSM 3 was made aware at the beginning of the school year of an incident involving CCSM 1 smacking Child F’s hand by Witness 1. During the interview with Child D’s Mother, she stated about a month ago, Child D informed her partner that CCSM 1 “smacked” a child. During the interview with Child F's Mother and Father, they reported Child F told them CCSM 1 smacked her hands and put her in the hallway because she was misbehaving.

The licensee designee failed to report an allegation involving inappropriate contact to the parent and the department. CCSM 3 reported when she was made aware of the allegation that CCSM 1 "smacked" a child by Witness 1 she completed an internal investigation. Once CCSM 3 determined they allegation was unfounded it was not reported to the parent or department. Witness 1 reported being made aware of the incident of CCSM 1 allegedly smacked a Child F by Child F's parents. He immediately contacted CCSM 3 regarding the allegations. Child F's parents did not report how they became aware of the CCSM 1 allegedly smacking Child F. Child D's Mother stated Child D informed her partner that CCSM 1 smacked a child.
