The injury to Child A was not a result of the center not providing appropriate care and supervision. Child A tripped on her own feet and fell forward on her arm while in the hallway during arrival time.

R 400.8131(1) · R 400.8131(1) Professional development requirements. The center shall provide an orientation about the center's policies and practices and these administrative rules for all personnel hired after the effective date of these rules and before unsupervised contact with children.

CCSM1 reported that she had not been trained on the center's policies or procedures on how to handle incidents of lost/unattended children. The center did not have documentation on file to verify CCSM1 received an orientation on the centers policies and practices or the administrative licensing rules.

A center shall maintain an accurate record of daily attendance at the center that includes each child's first and last name and each child's arrival and departure time. Electronic records may be used. If electronic attendance records are used, then they must be available to the department at the time of an inspection. If the electronic attendance records are not available during an on-site inspection, then the center is in violation of this rule.

The center ccsm's failed to maintain a record of accurate daily attendance that had dates of attendance as well as arrival and departure times when the electronic attendance method was inoperable.
