    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    Ms. Taylor reported that Child A's Mother left a voice message for Ms. Taylor on July 27, 2022, at 7:30 pm after the center closed that Child A had a bruise on his back. She contacted Child A's Mother the same evening and advised her that she would talk with staff to see what she could find out. Child A's Mother advised that she send pictures to Ms. Taylor. Ms. Taylor interviewed staff the following day and staff denied being aware of witnessing Child A get injured. When Ms. Taylor received the picture from Child A's Mother it was a picture of the fatty part of his arm between the chest and underarm area and not of the back. Ms. Taylor described the mark as a rash or prickly bumps. She asked Child A's Mother for clearer pictures and asked about the pictures of Child A's back. Child A's Mother has not communicated further and he has not returned to care. On August 5, 2022, i interviewed Child A's Mother. Child A was picked up from the Mother's center on July 27, 2022, by Child A's wife. During bath time Child A was observed with what was described as two thumb print bruises on each of his shoulder blades. She contacted the program director Ms. Taylor inquiring what happened to Child A. Ms. Taylor seemed annoyed that Child A's Mother was contacting her after hours on her personal cell phone_ She denied that anyone reported to her any injuries to Child A and that she would speak with staff and follow up with her the following day. She informed Ms. Taylor that he was sent to the center with no bruises and she is expecting an explanation on how the injuries occurred. The following day when she spoke with Ms. Taylor via telephone she felt as if she was given inconsistent information. Ms. Taylor reported that possibly it was a school-age child as they had been having issues with children picking up children and rough housing. She told Ms. Taylor that this was unacceptable and that the children should have been supervised. She overheard in the background saying the achoolwage children were not in the buildi TThey both could be heard debating back and forth over if the school-age children were present in the building. At some point in the day Child A was sent to the school-age room for care. Child A's Mother denied that she signed a waiver for Child A to be in the room with school-age children. Child A has given his mother inconsistent information on how the injuries occurred. He initially reported that the injuries occurred from Child B male agee . The story then changed to child care staff member ccsm him. "I would sale my soul before I believe did it. My child has a high. imagination and his story keeps changing. l do not know what to believe." Child A's Mother placed her phone on the speaker and asked Child A what happened to his shoulder area. "l was playing outside and we were squirting water at each other. They squirted water back at me and he threw me against the wall. It was a kid but I do not know his name." Child A's Mother told this consultant that this is the third version of what happened. The day of the injury the marks were red but the following day the marks had darkened and were bruises and you could clearly see thumb prints/marks according to Child A's Mother. On August 5, 2022, Child A's Mother submitted pictures via email to this consultant. Child A's Mother submitted two pictures to this consultant. The first picture (taken on the day of the incident) was reviewed and I observed two red marks on opposite sides of his shoulder blades in the same spot. There were areas on both sides of the shoulder blades that looked red and irritated. The picture looked as if there was a mark on Child A's neck. Child A's Mother denied any marks on his neck and reported that it was just shadowing from the camera. She also denied he had any bruises on his back despite initial reports that the back had bruises. Child A had scratches/ scrapes on his back. She reported that those marks looked old so not sure if those occurred at home an
