There was a child who has an Epi pen medication and the center was unable to provide medical permission for administration documentation during the onsite inspection.

An Epi pen for an enrolled child was not in the original prescription box containing the pharmacy label with the dosing amounts, physician or the child's name.

There was a child who had an Epi pen at the center with an expiration date of 6/2024.
