Program staff did not provide appropriate care and supervision when they allowed children to play on two Little Tikes slides in the gymnasium. The two slides were not made for commercial use nor were they made for indoor use. Three children were observed standing on the slides and walking up and down the slides; three children were observed walking up the slide; one child was observed sliding headfirst down the slide; and one child whose pants were too long and covered her feet, was observed going up and down the slide, causing the potential for her to trip and fall. In addition, the slides were sitting on the hard gymnasium floor minus any type of padding. Program staff also did not provide appropriate care and supervision during lunch and rest time when they allowed children to stand while eating and sit on their feet. Children were also observed unsupervised at the napping area jumping on the napping cots.
