On March 15, 2021, I conducted an unannounced onsite inspection and arrived at 11:30 a.m. I observed all the rooms that were currently open—the infant room, the young toddler room, the older toddler room, and the preschool room. The children were all in their classrooms with appropriate care and supervision. Throughout the entire visit, there were no children seen running out of their classrooms without a child care staff member. The child care staff members were all seen appropriately engaging with the children and mindful of the children's whereabouts while in the classrooms. On March 15, 2021, I interviewed Licensee Designee/Program Director Michelle Rabideau. Ms. Rabideau explained that she has not witnessed any concerns with inappropriate care and supervision with her child care staff members. Ms. Rabideau stated that if she observed any of her child care staff members not providing appropriate care or supervision, she would address it immediately and provide assistance where it is needed. On May 10, 2021, I interviewed child care staff members Amanda Fackler, Jasunique Johnson, and Christine Osborne. Ms. Fackler, Ms. Johnson, and Ms. Osborne all expressed that they have not had any concerns or issues with lack of supervision at the center with the children. Ms. Fackler, Ms. Johnson, and Ms. Osborne all explained that they have also never witnessed any children at the center running out of the classrooms unattended. On May 11, 2021, I interviewed child care staff members Jaymie Lovelace and Marjorie Colwell. Both Ms. Lovelace and Ms. Colwell explained that they have not seen any issues with children running out of the rooms unattended at the center. Ms. Colwell stated that there have been a few child care staff members that no longer work at the center who were not as attentive, but no current issues or concerns in regards to supervision. On May 11, 2021, I interviewed Child A's Mother, Child B's Mother, Child C's Mother, Child D's Mother, Child E's Father, and Child F's Mother. The parents all expressed that they have never witnessed children in any of the rooms run around unattended, and that the child care staff members are always providing appropriate care and supervision with the children at the center. 3

The infant room had a total of six infants with two child care staff members, Amanda Fackler and Jasunique Johnson. The young toddler room had a total of six children with two child care staff members, Requlle Smith and Jaymie Lovelace. The older toddler room had a total of six children with two child care staff members, Christine Osborne and Kylie Capaldi. The preschool room had a total of six children with one child care staff member, Marjorie Colwell. During the onsite inspection, all the classrooms were in compliance with their required ratio of child care staff members to children. Also, I reviewed the center's children's and child care staff members attendance records for the months of February 2021 and March 2021, and I did not find any out of ratio concerns. On March 15, 2021, I interviewed Licensee Designee/Program Director Michelle Rabideau. Ms. Rabideau explained that she has been trying her best to maintain child care staff to children required ratios because the center was previously cited for this issue. Ms. Rabideau expressed that she is mindful of the numbers and as recommended in the previous investigation with these ratio concerns, the center will 4 temporarily close if for some unexpected reason they do not have enough staff coverage. On May 10, 2021, I interviewed child care staff members Amanda Fackler, Jasunique Johnson, and Christine Osborne. Ms. Fackler, Ms. Johnson, and Ms. Osborne all expressed that they have not had any concerns or issues in regard to the classrooms being out of ratio. Ms. Fackler, Ms. Johnson, and Ms. Osborne all stated that they feel supported by Ms. Rabideau and have not had any issues. Ms. Facker stated all child care staff members do their best to help whenever there is a need for coverage and their volunteers have also been a help when needed to ensure the center is in compliance with ratio. On May 11, 2021, I interviewed child care staff members Jaymie Lovelace and Marjorie Colwell. Both Ms. Lovelace and Ms. Colwell explained that they have not seen any issues with the rooms being out of ratio. Both Ms. Lovelace and Ms. Colwell stated that things have improved greatly since the last complaint was filed against the center for out of ratio concerns. On May 11, 2021, I interviewed Child A's Mother, Child B's Mother, Child C's Mother, Child D's Mother, Child E's Father, and Child F's Mother. The parents all expressed that they have never witnessed the classrooms out of ratio. Child C's Mother, Child D's Mother, Child E's Father, and Child F's Mother all stated that there were a couple times in the recent past where they were notified that the center had to close temporally due to not having staff for that day and were appreciative that the center chose to do that instead of remaining open.

On Marcy 15, 2021, the preschool room had a total of six children with one child care staff member, Marjorie Colwell. The room was within ratio of children to child care staff members. On March 15, 2021, I reviewed center's children and child care staff members attendance records for February 2021 and March 2021. I did not observe any concerns with ratio.

On October 15, 2020, I initiated my investigation by conducting an unannounced onsite inspection. I arrived at 3:00 p.m. I interviewed Program Director/Licensee Designee Michelle Rabideau. I explained that the department received information that appropriate background checks were not being completed on the volunteers that assist at the center. Ms. Rabideau stated that her volunteers are members of Redeemer Lutheran Church and have been a tremendous support to the center. Ms. Rabideau explained that she never leaves volunteers alone, and that they are always in the room with one of the child 4 care staff members. Ms. Rabideau further stated that all volunteers have been appropriately background checked per child care licensing requirements. The three supervised volunteers that come to assist regularly at the center include Volunteer 1, Volunteer 2, and Volunteer 3. Ms. Rabideau provided volunteer files where I was able to review and verify that supervised volunteers Volunteer 1, Volunteer 2, and Volunteer 3 all had their public sex offender registry clearances completed on July 16, 2020. In addition, Volunteer 2 also completed the comprehensive background check fingerprinting with documentation verifying that she was found eligible and able to be left unsupervised. On December 10, 2020, I interviewed volunteers Volunteer 1, Volunteer 2, and Volunteer 3 who all expressed that they are always in the classrooms with another child care staff member when they volunteer.

During the investigation, Child Care Staff Member 1, Child Care Staff Member 2, Child Staff Member 3, and Child Care Staff Member 4stated that they have observed and witnessed the Program Director/Licensee Designee Michelle Rabideau not provide positive methods of discipline to some of the child care children.
