Child A (female,,years old) told her mother that punched her in the stomach after Child A bit her. This incident occurred between 7/1512022 and 7118/2022. cognitive delays, protocol was not able to be established. Child A's Mother reported that she took Child A to the doctor after the incident because Child A was complaining about her back hurting after being hit by Child A's doctor was contacted and reported that he saw Child A on 8/02/2022 and she had no marks or bruises at that time. Child A has two siblings Child B (maleUyears old) and Child C (female, $years old) that were also enrolled at the child care center. and I spoke with center director Misty Maniwczak via telephone. explained that an investigation was assigned. Ms. Maniwczak re p orte Favingag a child care staff member (CCSM) employed by the name of Ms. Maniwczak reported that she did not submit an incident report to child care licensing regarding the allegations. On 08/11/2022, and I made an unannounced on-site inspection and spoke with Ms. Maniwczak. Ms. Maniwczak was informed of the allegation and explained that she was not working on 7/18/2022 but had been informed the following day that Child A's Mother was asking because Child A told for- her that "beat her." Ms. Maniwczak reported that the children had only been attending the center for two weeks. At enrollment, Child A's Mother told her that Child A had . but Child A had Child A was lifting uo the wouia oniv nave contact w infants in the hallway and after she grabbed at their legs and tried to hit them, she was redirected but laughed as if it was a game. Child A's behavior was better when Child C was also present, but Child C was not always present. CCSMs were crying and did not want to come to work because of Child A's behavior towards them. CCSM is the lead caregiver in the classroom and told Ms. Maniwczak that Child A has said "you beat me, you beat me." CCSM called her to talk about Child A's behavior. Ms. Maniwczak said that because "ewas off work on 7/18/2022 and had received multiple complaints from CCSMs, she asked CCSM to talk with Child A's Mother about her behavior that day. On , Child As Mother came to the center and told that Child A had told her that beat me." was cornered in the kitchen by Child A's Mother w o was yelling at her and asking to speak to Ms. Maniwczak spoke with Child A's Mother on 7/20/2022 and informed er at Child A was disenrolled due to her behavior. Ms. Maniwczak talked to Child A's Mother about the accusation against during the phone call. Ms. Maniwczak C1 did not report the accusation to licensing or because she thought she was only supposed to report if she suspected the allegation was true. She said that she did not believe that anything happened to Child A and that was not caring for her. was working in the preschool classroom that day and would have only had contact with Child A on the playground when the classrooms combine at 4:30 PM. I asked to see the attendance records for Child A. Ms. Maniwczak reported that she did not have attendance records for Child A for 0711812022 because her mom did not sign her in and out in the ProCare system they use for electronic attendance. I also reviewed the enrollment paperwork for Child A. The paperwork had a section for_", and it was indicated that Child A needed, sometimes repeat get hat & coat" and the accommodation she needed "reminders." was Ru and I interviewed CCSIVI who reported that she learned about the allegation against her from Ms. Maniwczak. told her that Child A's Mother was asking for her and had cornered her. said that she was told that Child A claimed she physically abused her. denied the allegation and said that she worked in the preschool room that day. Child A was with and was touching her. needed a break, so Child A was sent outside with who was on the with four preschoolers. Child A was applying sunscreen and told her that she had applied enough. On 10/01/20, I spoke with Child A's Mother who she stated that on 09/17/20 her husband was told by child care staff member, Melissa Hill, that she took their son for a long walk. Her husband asked her when he got home if she had given the center permission to take their son off the premises. She said, "no". She said that she told her husband that maybe Ms. Hill meant that they walked around the premises. However, when she spoke to the director, Ms. Misty Maniwczak, Ms. Maniwczak informed her that she gave child care staff member Melissa Hill permission to take Child A for a walk in the surrounding neighborhood. Child A's Mother stated that she then told Ms. Maniwczak that she did not give permission to allow their son to be taken off the premises. Child A's Mother stated that Ms. Maniwczak replied, "no, you're right", "I should have gotten your permission". Child A's Mother told her that moving forward she does not want this to happen again and that she was not okay with it. Ms. Maniwczak assured her that it would not happen again. On 10/15/20, I conducted an unannounced on-site inspection. I spoke with Director, Misty Maniwczak regarding the allegation. After reviewing the allegation with Ms. Maniwczak, she stated that she has been working at the center for 12 years and ever since she has been here, they have taken the infants for a walk around the neighborhood. They never had anything in writing allowing them to do so. However, after Child A's Mother complained, she thought it might be a good idea to have a permission slip. She has since created a walking permission slip and had all the parents sign one. I also spoke with child care staff member, Mariel Palen about the allegation. She stated that she has been working at the center for almost a year and for as long as she has been employed at the center there has never been a permission slip signed to walk the infants in the neighborhood. She stated that the mother of one of the infants was upset about it. However, since that incident the center has create a permission slip. On 10/15/20, I spoke to Melissa Hill regarding the allegation. Ms. Hill stated that on the day the she took Child A out for a walk, the director Misty Maniwczak came into her room and told her that it was nice out and then suggested that she take the infants for a walk around the neighborhood or on the bike trial. She said that she only had 3 infants, so she took them in stroller around the neighborhood. She said she took them for about a mile walk. She stated that when she returned to the center Misty Maniwczak told her that Child A needed to be fed. When Child A's Father arrived, she told him that they had just gotten back from a walk, she finished up feeding him and then changed his diaper. Shorty after Child A and his father left, Child A's Mother called Misty Maniwczak to tell her that she was upset that her son 3 had been taken off the premises without her knowledge or permission. After Ms. Maniwczak spoke with Child A's Mother, she then told them that the Child A's Mother was upset that they took him for a walk in the neighborhood without her knowledge or permission and she then instructed them not to take him back out. Ms. Maniwczak also told them that she would need to get permission slips from parents. Shortly thereafter she developed one and had all parents sign them. Ms. Hill stated that Child A never returned after that incident. On 10/13/20, I spoke to Child D and Child E's Mother regarding the allegation. She stated that she is sure that they have taken her children for a walk however she does not remember signing any sort of permission slip at the time of enrollment but remembers recently signing one within the last two or three weeks. On 11/13/20, I spoke with Child B and Child C's Mother regarding the allegation. She stated that she knows staff members take the infants for walks and she never had a problem with it. She said she never thought about whether she signed a permission slip or

I spoke to Ms. Maniwczak about the allegation regarding the feeding and handling of bottles by child care staff member Melissa Hill. Ms. Maniwczak stated that she is not aware of Ms. Hill not following the feeding rules. She stated that she knows that Ms. Palen and Ms. Hill do not get along very well, and she has been trying to address them regarding their working relationship. I asked Ms. Palen if there were any issues or concerns regarding feeding of the infants. She stated that August 31, 2020 was the first time that she noticed Melissa Hill started feeding Child B shorty after 1p m. Melissa Hill logged in the app that she gave Child B 2oz and then at 2:39pm Melissa Hill put in the app that she fed Child B another 3oz. She stated that Child B only had two 5-ounce pouches for the day. She gave her one in the morning and then Melissa Hill gave her 2oz at right around 1p m and then the other 3 oz at 2:39pm. Ms. Palen stated that her concern is that Melissa Hill took longer than the hour allowed to feed Child B and then lied about it when she documented on the "hi mama" app. I also spoke with Ms. Hill about the complaint. I asked Ms. Hill if there was a time that she took longer than the hour allowed to feed Child B but documented it inaccurately in the "hi mama" app. Ms. Hill denies that on 08/31/2020 she feed Child B the same breast milk, for over an hour. She said she knows that she cannot feed infants the same bottle if it has been longer than one hour since warming the bottle. She also denied that she documented inaccurate feeding times in the "hi mama" app. She indicated that she knows that Ms. Palen is the one that is saying that, but she does not know why. She said that Ms. Palen has had a problem with her ever since she started in the infant room and she is not sure why. On 11/20/20, I spoke with Director Misty Maniwczak regarding the allegations. I asked Ms. Maniwczak if she could provide a copy of the daily sheet from the "hi mama" app for the day of 08/31/2020. She stated that she would figure out how to get it and email mail to me. Shortly after speaking to Ms. Maniwczak I received an email from her of Child B's daily schedule for the date of 08/31/2020. On that day the "hi mama" app confirms that Child B was fed 4 oz in the morning and then 2 oz at 5 12:59pm and 3 oz at 2:38pm. However, it does not indicate how many bottles were brough in for Child B. After receiving the daily sheet from the center. I completed a follow up phone call with Child B's and Child C's Mother regarding her daughter's feeding on 08/31/2020. She stated that when her daughter initially began attending the center, she was bringing the bottles thawed and correctly labeled for the first week and then by the middle of second week she started bringing them frozen. But she does not remember how many bottles she brought on the first day. She believes it was 2 or 3 but she cannot say for sure. She said that sometimes it depended on if her daughter ate before she was dropped off or not. Also, she not totally sure because her husband would mostly take her and pick her up and she not sure how many bottles he would put in the bag. I asked her if she could speak with her husband to see if he knows how many bottles he packed for her. She stated that she would speak to him after work and let me know. I later received a text message from Child B and Child C's Mother indicating that she spoke to her husband and indicated that he could not recall how many bottles he sent in that day, but he believes it was either 2 or 3 but he is not for sure. She said that they always send in between 2 or 3 bottles.

Child A's Mother stated that on at least once or twice a week she would get her son home and he would not have any of his empty bottles or sometimes only one empty bottle would be returned to her. When she would send the director Misty Maniwczak a message about Child A's bottles. Misty Maniwczak would say, "oh, I'm sorry we must have forgotten" or "I'm sorry we will look for them" Child A's Mother also informed me that child care staff member, Mariel Palen told her that Melissa Hill inaccurately entered information regarding an infant's feeding in the "hi mama" app, indicating the infant was fed three bottles. She was also told by Ms. Palen that the following Monday morning, 09/28/2020, when she was putting the bottles away for the day, she still saw two bottles in the freezer leftover from Friday. Ms. Hill stated that on 09/25/2020 she feed Child B in the afternoon. She does not know how any bags of breast milk where brought in that morning. She does, however, admit on Friday she forgot to send the rest of the frozen breast milk home, but she gave it to the mother on Monday. Ms. Maniwczak stated she did not have concerns with Ms. Hill not documenting accurately in the "hi mama" app. She stated she knows that on most days the staff will return to parents all the unused bottles and frozen breast milk. She stated that she has not had any parents contact her with any concerns. On 10/13/20 @ 5:15pm, I spoke to Child D and Child E's Mother regarding the complaint. She stated that she hasn't had any major concerns however there were a couple times when her son had one bottle left and she felt he shouldn't have but she wasn't really concerned about it because he is also on solid food's. She simply thought that they must have given him solid foods instead of another bottle which would have been fine. She never questioned it but now she wonders if she should have. On 11/13/20, I spoke with Child B and Child C's Mother regarding the complaint. She stated she had not had any concerns with her daughter's feeding or milk. She stated that she was not sure what the staff member was referring to regarding her daughter's milk or feeding but she cannot recall having any issues or concerns about it. She said as far as she knows everything reported in the "hi mama" app has been accurate. She said her daughter has been fine when she picks her up and appears to be fed and changed so she never had any issues or concerns. She said that she does not really remember a time when the left-over breast milk was not returned to her, but she said that it's possible that this happened, but it was a long time ago and she doesn't remember for sure.

Melissa Hill, child care staff member, fed an infant a bottle that had been sitting out for longer than one hour. On 10/05/2020, a second allegation was received indicating that on 08/31/2020, Ms. Hill warmed a bottle at 12:45pm and rewarmed the same bottle at 2:39pm. Ms. Hill is not feeding infants but telling parents that she is on the electronic care tracking app. On 10/05/2020, a second allegation was received alleging that on Friday, 09/25/2020, Child B was not fed according to the electronic care tracking app. She had three containers of breast milk; however, two containers were still at the center on Monday morning. The tracking app indicated that she had been fed three times throughout the day.
