All child care staff members who work directly with children are required to be trained in first aid and pediatric, child, and adult cardiopulmonary resuscitation (CPR) within 90 days of being hired. Prior to issuing a license to operate a child care center, and prior to the renewal of a license, the department shall verify that at least 50% of the child care staff members who work directly with children are currently certified in first aid and pediatric, child, and adult CPR. Each of these child care staff member's first aid and CPR certificates must be valid a nd retained on file in the center.

Based on the on-site inspections and interviews with parent of children in care licensee Ms. Salinas- Maldonado does not have the administrative capability to provide services that are conducive to the welfare of children. This is the second special investigation that resulted in a child leaving the facility, SIR 2023D0910017 Dated 01/22/2024. Special Investigation 2023D0910017 initially recommended license revocation; however, a settlement agreement was reached 2/24/2025, resulting in the facility being placed on a first provisional license. Ms. Salinas-Maldonado remains within the provisional licensing period and has had another incident involving a child who left the facility while in care. This special investigation recommended a revocation; however, a settlement agreement was reached, 2/24/2025 and the center placed on first provisional license. Ms. Salinas- Maldonado is currently in her provisional period and has another child that has left the facility while in care.

On 04/01/2022, I spoke with Child A's parents. They both stated that on 03/29/2022, Child A's Mother was notified that Child A hair had been pulled out when she arrived to pick Child A up. Child A told her mother that Child B (1-year- old, male) pulled her hair. Neither parent was notified by the child care center. Child A's Mother stated that Victor Reyes did not show any concern. Child A's parents are concerned that the children are not being properly supervised. A police report was filed with the Southgate Police Department. The parents had not received a copy of the police report. According to Child A's parents, this is the second time there was an incident with Child A hair being pulled out. The first incident was on December 17, 2021, the parents were not notified. Child A hair was pulled out but not the entire braid. On 04/01/2022, I received an email from Child A's Mother. There were photos of Child A's hair where the braid/hair was missing from the incident on 03/29/2022 and 12/17/2021. There was a copy of the accident reports that the child care center gave to Child A's Mother. The incident report was signed and dated by Victor Reyes on 12/17/2021 and the second incident report was signed and dated by Victor Reyes on 03/30/2022. Both incident reports indicated that the parents were not notified of the incident. The incident report signed on 12/17/2021, stated, that incident happened on 12/10/2021, the time of the incident was unknown. The report stated that Child A was playing by herself all day. Child A did not call for help, cry or scream. According to the incident report Child A was not injured in the child care center. The incident report signed on 03/30/2022 stated that incident happened on 03/29/2022, the time of the incident was left blank. The report stated that Child A hair was playing with Child B. Child A stood up and Child B grabbed her hair and pulled her down by her hair. This resulted in her braid detaching from her scalp. On 04/11/2022, I completed an unannounced onsite inspection. I spoke with Cintia Carreon, Child Care Staff Member (CCSM). She stated that she was not working in the classroom when the incidents occurred with Child A. She told me that I would need to speak with Victor Reyes. On 04/11/2022, I spoke with Victor Reyes, CCSM. He stated that the first time the incident happened with Child A hair being pulled out, no one witnessed the incident. The incident happened a couple months ago, Child A never cried. The second time the incident happened in March 2022. Victor Reyes stated that he was supervising the children. Child B reached and grabbed Child A's hair and pulled her down. Child B was trying to get Child A to keep playing. There has never been an incident before where Child Bhas pulled a child by their hair. 3

On 04/01/2022, I spoke with Child A's parents. Both parents said, Child A's Mother was notified that Child A hair had been pulled out when she arrived to pick Child A up. She was not called when the incident occurred. On 04/11/2022, I spoke with Victor Reyes. The incident happened at approximately 3:30 p.m. He was waiting for Child A's Mother to arrive to notify her of the incident. When Child A's Mother arrived, he spoke with her and notified her of the incident. He also notified Child B's Mother. Mr. Reyes did not attempt to reach Child A's parents when the incident occurred. On 04/12/2022, I spoke with Ms. Maldonado. She said she would send me the policy on reporting child incidents and the child information card for Child B. On 04/12/2022, I received the policy for reporting incidents with children. The policy indicated that if an emergency arises on or off Little Steps Youth Learning grounds, the center's staff will first administer first aid, contact the parents, and notify the family physician. If the parents cannot be reached, the staff will contact the emergency numbers provided by the parents.
