On 2/17/2026, the required ratio of 4:1 was not met in the toddler classroom when the first child woke up during nap time. There were seven children present with one teacher. Three of the seven children were awake in the toddler classroom during observation. The toddler room was out of ratio over 15 minutes.

On 09/01/2020, I called the center and spoke with the assistant director, Lisa Ammons. She confirmed Child A were present at the center. The program director, Anne Kostecki, was not present at the center. On 09/08/2020 I conducted an onsite inspection at the center. Mrs. Ammons confirmed that both the lead caregiver, Lisa Ammons, and Child A were in the building and available to be interviewed/observed. The program director, Anne Kostecki, was out running an errand. Mrs. Ammons said she was aware of the incident as it was told to her by Mrs. Kostecki and the lead caregiver, Audrey Pigula. Ms. Pigula has been employed at the center for several years and has no history of inappropriate care or supervision. She has a signed child abuse/neglect statements on file at the center and her file is free of written complaints regarding inappropriate care or supervision. I observed Child A in his classroom with three other students and two caregivers. I interviewed lead caregiver, Audrey Pigula, while the center. She said Child A and Child B were in close proximity to each other when they collided. First Aid was administered to both children. At that time, Child A was the only one who was injuried. Abovehis eye was red. Child B was consoled and his parent was notified. Ms. Pigula said many of the young toddlers are still working on walking and may fall often. She said both children were going for the same toy when they hit heads. She denied lack of training regarding supervision and appropriate care. She also denied that ratio is a problem at the center. On 09/13/2020 I returned a call to Mrs. Kostecki. She said she was had no idea Child A's mother had any concerns. She said she spoke with her the two days following the incident and Child A's mother even said she was thankful for their services. They have always had a working relationship. Mrs. Kostecki said a local police officer visited the center three days after the incident saying a complaint of physical abuse had been made. She said she was shocked as Child A's mother continued to drop of her children at the center. She said she has no concerns regarding her staff members and their ability to provide appropriate care. She said all of her staff are well trained and are long standing caregivers. On 09/22/2020 I conducted a follow-up inspection. I observed the Toddler room children and their caregivers. I saw no concerns of inappropriate care and supervision. Both caregivers were attentive to the need of the children. On 10/30/2020, I conducted a telephone interview with Child A's mother said she took her children out of the center on 09/27/2020 due to continued concerns of "abuse". She said the caregivers first told her Child A ran into another child then later 4 said he sustained a carpet burn. She said there were incidents with her other child, but she did not really "put it together. She said once Child A started getting hurt, that was it. She began to take it personally. Child A's mother said Child A was "given pink eye by the center" so she took both of her children out on or about 09/27/2020. She said she does not know what the center had against her and her children, but she could not continue to let them get hurt. There was also an incident when Child A was going in the center from the playground and fell outside on the concrete. She wanted to raise the concern for other children that will continue to attend the school. On 11/10/2020 I interviewed Child B's mother over the phone. She said she was made aware of the fall and Child B did not sustain any injury. She had no concerns regarding inappropriate care and supervision at the center. Child B has been attending the center for 5 months. On 11/10/2020 I interviewed Child C's (9-month-old female) mother over the phone. She had no concerns regarding inappropriate care and supervision of her child. She has two children that attend the center and has had no concerns.
