    On 02/02/2023, I spoke with the complainant. No additional information that could be supported by a rule was provided. On 02/02/2023, I spoke with Child A's mother. She shared Child A enrolled into care at the facility on October 19, 2022, and was withdrawn on December 8, 2022. On November 3, 2022, Child A came home from care with two small red areas behind his right ear. She sent Ms. Alexander pictures and asked if anything occurred while Child A was in care. Ms. Alexander reported having no knowledge of Child A having the red areas behind his ear and she did not notice them while he was in care. She shared an incident report was never received for this occurrence. On November 28, 2023, Child A was picked up from care by his father with two small knots on his forehead. She shared his father did not notice the small knots and it wasn't until Child A returned home that she noticed them and called Ms. Alexander, at which time Ms. Alexander told her she forgot to tell us about them. She did not know how Child A had the two small knots on his forehead and said maybe it came from his napping on the cot because she didn't notice it until after he woke up from his nap and was picked up by his father shortly afterwards. Child A's mother shared an incident report was never received for this incident. On 02/03/2023, I completed an unannounced onsite inspection. I spoke with program director/designee, Loretta Alexander, and child care staff member (CCSM), Taya Alexander. Ms. Alexander expressed frustration with the complaint and shared she believed the complaint was called in by a disgruntled parent who withdrew their child weeks prior. Ms. Alexander confirmed knowledge of Child A's enrollment and when he was 3 withdrawn per the parents' request. She said that while Child A was in her care for approximately 1 months, she nor her staff ever observed Child A to have any 1/2 incidents that resulted in injuries. There was a time when Child A woke up from nap, and while she was transitioning him to the restroom, she noted two small swollen spots on each side of his head. Child A's parent came to pick up Child A within minutes of this, and while the parent was gathering Child A's things, she was speaking with another parent and did not get an opportunity to mention to him her observations. She recalled receiving a call from Child A's mother shortly after Child A returned home. She told the mother she had just noticed the two small spots and wondered if he hit his head on the cot or if it was some kind of skin reaction. When Child A arrived that day, she noticed he had a skin rash on his stomach and legs, and she contacted the mother who said she believed he had a reaction to the laundry detergent. With that knowledge and Child A never having any incidents during the day, Ms. Alexander wondered if it was just a skin reaction. Ms. Alexander stated she worked with Child A in the classroom during the day of this incident. An incident report was never written up because she had just noticed it at the same time the child was being picked up and because an incident never occurred while he was in care. Ms. Alexander shared she takes pride in how well she communicates with her parents, and if I were to speak with them, they would all share she communicates constantly. Ms. Alexander shared Child A continued to come to care for two weeks after this and the parents didn't express any safety concerns or concerns for Child A being in her care. She believes Child A's parents became upset during a phone call when she noticed Child A came to care with a blue tongue and his behavior was "very hyper." She called Child A's mother, and she was offended that she asked if he was provided candy due to his behavior while in care. She shared this question was only asked because she was told by Child A's mother Child A was seeing a behavioral therapist due to behavioral concerns, which she immediately noticed upon enrollment and during this day he was having to be redir
