Child A B's Mother stated on 05/01/2023 after she icked the children up from child care, she noticed on the Child A's Child A did not have a wenssh e was dropped e child care center on 0510112023. Child A didnot have a nor was she ' after she picked her up from the child care center on 05/01/2023. On 23, she went to the child care center and asked about the to Child A. Ms. Montgomery told her that a CSSM, whose name "Se could no reca , when she changed her diaper and noidRild A and Child B's Mother stated someone froni the center should have called her immediately if they noticed Child A wa The center did not give her an incident report regarding the . The center gives her a written daily record for the children, and they also use the Brightwheel app to update her on the children's food intake, sleeping patterns, diaper changes, and other information. She is unsure if the written daily record that she received on 0412812023 and 05/01/2023 indicated any for Child A because she stated she never reads the written daily records that the center gives her. The Brightwheel app did not indicate any for Child A. Child A and Child B's Mother does not believe that there was Child A's . She believes the child care center is stating that owas, a ie o hild A to cover up that the CSSaMn,y wWhoocchanged her, The children have been attending the child care center about a month. Their endance has been sporadic, and they attend child care one week and may not attend the next depending on Child A and Child B's Mother's child care needs for that week. She noticed since Child A and Child B began attending the child care center, Child A has been fussy when she changes her diaper and is clingy to her when she drops her off at the center, which is abnormal behavior for her. Child A and Child B were taken to the hospital for exams and neither child had a diaper rash or bleeding. The child care center's policy states that only prescription diaper rash cream can be applied. She never gave the child care center diaper rash cream for the children. She believes she may have signed a consent form for the child care to apply diaper rash cream, but she is not certain. The children will not be returning to the child care center. She has no other concerns regarding the child care center. On 05/04/2023, 1, along with , interviewed Ms. Montgomery during an unannounced on-site inspection at the center. Ms. Montgomery stated on 0412812023 CSSM 1 asked her to go into the classroom to look at the that she observed when she changed Child A. She observed a every time CSSM 1 wi ed Child A, and she also observed a There was not a lot , only a that could be seen after she had wiped her. The on the wipe came from the , however; she was uncertain what caused th The was not observed when she was changed later in the day. DiaperRrascream was not applied during Child A's diaper changes on 04/28/2023 and 05/0112023. The child care center only applies diaper rash cream when a parent provides a prescribed diaper rash cream and when a parent signs a permission form allowing the child care center to administer the diaper rash cream. Child A and Child B's Mother did not provide the center with any prescribed diaper rash cream, and she did not sign a permission form allowing the center to administer diaper rash cream. Friday was the first time Child A and Child B attended the child care center as they had not attended the child care center for about two weeks. After observing the Ms. Montgomery told CSSM 1 to document the Child A's written daily record. The center no longer uses the Brightwheel app to document the daily record because she determined it was a distraction to staff, as they would stay on their cell phones after entering information on the app. Child A and Child B were picked up by their aunt the evening of 04/2812023 and Ms. Montgomery and CSSM 1 had already left the center for

The center only administers diaper rash cream when it is prescribed by a doctor and the parent consents to the center administering it. Child A and Child B's Mother did not provide the center with a prescribed diaper rash cream, and she did not sign a consent form allowing the center to administer diaper rash cream. It cannot be determined that the center applied diaper rash cream on Child A. Although Child A and Child B's Mother stated she observed diaper rash cream on Child A's area, the staff denied administering diaper rash cream on Child A.

On 10/06/2021, I spoke with Child A's mother via telephone. On 09/17/2021 at around 12:30pm, Child A's mother received a telephone call from the program director, Ms. Montgomery. Ms. Montgomery informed her that Child A was hit in the face by a chair thrown by Child B, causing Child A to sustain a "busted lip". Child A's mother immediately went to the center to pick up Child A. When Child A's mother arrived at the center, she observed a laceration to Child A's lip. Ms. Montgomery informed her that Child B has a history of hitting other children and staff 2 members. Child A's mother stated that this was the first time she was informed that there was a child with violent behavioral issues in the PreK classroom. She believes that this incident could have been prevented if Ms. Montgomery had removed Child B from the program, or at least watched Child B more carefully to avoid any contact with other children. Child A's mother stated that she did not take Child A to the doctor to be examined, as she felt the injuries did not require medical attention. Child A's mother was upset that the center did not follow up with her regarding the incident to inform her what measures were put in place to avoid this from occurring again. Child A's mother disenrolled Child A from the program due to safety concerns. On 10/12/2021, I made an unannounced on-site inspection. I interviewed Darniska Montgomery (program director) and Kimberly Dent (assistant caregiver-floater). Staff members were interviewed privately in the program director's office. Ms. Montgomery stated that on 09/17/2021 she was serving lunch in the PreK classroom. She observed Child A and Child B standing by the table on the other side of the classroom. Child B picked up a chair and swung it, hitting Child A directly in the face. Ms. Montgomery indicated that Child A was not doing anything at the time to provoke Child B, and it appeared that Child B deliberately hit Child A in the face with the chair. Ice was immediately applied to Child A's face and Ms. Montgomery promptly called Child A's mother and informed her of the incident. Child A's mother arrived at the center and took Child A home. Child A's mother was very upset by the incident and disenrolled Child A from the program. Ms. Montgomery said that Child B had past incidents of striking caregivers and children, but that none of these incidents had resulted in injuries. Child B's behaviors were mainly described as tantrums. For example, Child B would sometimes throw a chair or other items out of frustration, but never directly at other children. The center was working with Child B and the family on trying to come up with behavioral management techniques. Ms. Montgomery stated that all staff members were aware of Child B's past conduct and would often remove Child B from a situation when they saw her behavior escalating. Staff members would frequently sit by Child B during snacks and meals, and at large group activities, and they were usually able to redirect Child B and maintain the safety of all the children in the classroom. After this incident involving Child A, Ms. Montgomery disenrolled Child B from the center due to safety concerns. Ms. Dent stated that on 09/17/2021, she was in the toddler room, which has a half door looking out into the PreK classroom. Ms. Dent stated that she heard Ms. Montgomery yell out, "Why did you do that?". Ms. Dent did not witness the incident but saw Child B holding a chair. Ms. Dent stated that she was aware of Child B's history of tantrums and outbursts and had been working with the family to come up with solutions both at the center and at home. Ms. Dent denied that she was aware of any incidents where Child B had injured other children, and she characterized her past behavior as "acting out" and "unruly" as opposed to physically aggressive. Ms. Dent stated that the staff members were all aware that Child B needed extra 3 attention and would often position themselves to be near Child B to hel

Ms. Montgomery stated that the center always has enough food for all the children in attendance, and she typically has left over food after meals and snacks are served. Since the center is open from 6:00am-12:00am, the children are served breakfast, morning snack, lunch, afternoon snack, dinner, and evening snack. Ms. Montgomery denied receiving any complaints from parents or staff regarding the quantity of food being served to the children. Ms. Dent stated that she does most of the cooking at the center, and she usually has excess food left over after snacks and meals, as she routinely makes extra in case children are dropped off at odd times. She also indicated that the children may eat as much as they want and are never denied second servings. Ms. Dent stated that she cooks what is available in the refrigerator and freezer or what Ms. Montgomery instructs her to cook for the day. Ms. DeRamus stated that the center has enough food for all the children in attendance and denied that she has ever seen children not being fed all the required meals and snacks. Ms. DeRamus also indicated that no child or parent has ever complained to her about the quantity of food being served. She indicated that, once prepared, meals and snacks are served in the classrooms, and caregivers will often ask for additional food if the children eat everything that is initially brought in. The menu for that day was not posted, however, while I was conducting my inspection, Ms. Montgomery prepared a list identifying what snacks and meals were going to be served that day. The handwritten menu for that day was as follows: • Breakfast - Cheerios with milk • Morning snack - cheese puffs and milk • Lunch - hotdogs, sliced bread, canned peaches, vegetable medley • Afternoon snack - animal crackers • Dinner - hamburgers, sliced bread, peas, sliced apples • Evening snack - turkey and cheese sandwich I observed the kitchen, which had one refrigerator and one freezer, and both appeared fully stocked with food. The back-storage room had two refrigerators and one freezer which were also stocked with food. The pantry was full of dry goods. On 12/08/2020 Child A's mother stated that her child is enrolled at the center and is often in attendance at dinner time. Child A's mother expressed concern that they are not feeding Child A enough while he is in care because he always tells her he is hungry and asks for food when she picks him up at the end of the evening. 4 On 12/08/2020 Child B's mother stated that she has no concerns with the quantity of food being served to Child B. She indicated that Child B seems to like the food and has never complained about not getting enough to eat while in care. Child C's mother stated that the center provides a nice variety of food to Child C. She denied that she has ever had concerns about Child C not eating enough while at the center.

The morning snack served on 11/12/2020 only contained 1 of the required 2 components, since cheese puffs do not fulfill the grain requirement. The afternoon snack only contained 1 of the required 2 components.
