On 07/30/2021, I made contact with the referral source. No additional information was provided. On 07/30/2021, I spoke with Child A and Child B's mother. I discussed the allegations made against the facility. She reported having twins who attend the summer camp program at the facility. I inquired about documents provided during enrollment. She shared she didn't receive documentation other than verbal information about the camp and a flyer which let her know the themes of each week. 2 I specifically asked if a parent information packet was provided and noted some of the things this packet would include. She shared it was not. She shared, she specifically requested a schedule in writing via the HI Mama app on July 14, 2021, because she wanted a better sense of the daily routine. Ms. Melissa Jackson let her know that the promotional flyer for Camp Excellence that she received prior to registration was the daily schedule. I asked which classroom her children were in. She shared; Ms. Charlotte Woods is their assigned Child Care Staff Member (CCSM). On 08/05/2021, I completed an unannounced onsite inspection. I spoke with program director, Melissa Jackson. I informed Ms. Melissa Jackson was informed of the complaint made against the facility. I asked if parents are provided a written parent packet upon enrollment and discussed all the requirements identified in the rule (R400.8146). Ms. Melissa Jackson shared children enrolled in the summer camp program did not receive this packet. She shared she didn't think about giving it to summer camp enrollees. She does provide it to parents upon enrollment when they sign up for regular child care. I asked what parents receive during summer camp enrollment. She shared a contract, daily goals, and a weekly theme.
