    R 400.8137 · R 400.8137 Diapering; toileting. (6) Diapers and training pants must be checked frequently and changed when wet or soiled. 4

    On 9/20/21, Child A and B's mother said that she had not received any daily reports about the food intake, diapering or the sleep patterns for Child A. Child A and B's mother said that she did not receive any report for Child A or his sister Child B (2- year-old female) who attended the same center. Child A and B's mother said that Child B was 2 years old now, but she was not receiving any daily reports for her either. Child A and B's mother said that she did not know anything about a daily report to even ask the staff members for the same. On 9/27/21, I conducted an unannounced, onsite visit at the center at about 1:30 pm. Child C (6- month-old female) was present at the center. I reviewed the daily record for Child C. The entries were not made for diapering after 9 am. The food intake was not marked down after 10:45 am. The sleep time was marked as 10:45 am but wake up time was not documented, and Child C was awake. On 9/27/21, I interviewed Ms. Murray who said that she had instructed the staff members to document diapering, sleep, and food intake and since the staff member left in a hurry a few minutes ago for a doctor's appointment she did not document that. Ms. Murray said that nap time was when staff members documented all the times for food, sleep, and diapering. Ms. Murray said that they handed over the daily reports to all parents and she was not sure how Child A and B's mother did not receive anything. Ms. Murray said that they do not do daily reports for children over 1 year of age and she was not aware of the child care licensing rule for the same. On 10/18/21, I interviewed Staff member B and Staff member C who said that they did not do any daily reports for Child B since she was over 2 years of age. They said that they would only do daily reports for infants. Staff member B and Staff member C said that they maintained a daily report for Child A but sometimes would forget to make the entries. 5 On 10/18/21, I interviewed Staff member A who said that she never documented any food intake, diapering or sleep patterns for Child A since Child A and B's mother did not show any interest in the daily record. Staff member A said that none of the parents would ask for the record and she stopped doing them 2 months ago. On 10/26/21, I interviewed Staff member D who said that she always documented on a daily report for Child A and other infants, but she does not maintain a daily record for any children over 1 year of age. The center did not have any daily records for Child A. Child A was disenrolled from the program on 9/18/21. On 10/26/21, I reviewed the daily record for Child C who was present at the center. All the entries were made for the day.

    On 10/18/21, the department received a complaint stating that many times over the past 2 months, there was 1 staff member to 5 children and the youngest child in the group was an infant or a toddler. Complainant A did not remember the dates or the times but stated that this happened often. 7 During my inspection on 9/ e 3 children in the center. This included Child C, who was an infant Child D and Child E, who were preschoolers Ms. Murray was the only staff member present at the center. On 10/18/21, I interviewed Staff member A who stated that sometimes she had more than 4 children by herself, with the youngest child being an infant, while working at the center. She said that she was not sure of the dates, but this happened very often. On 10/18/21, both Staff member B and Staff member C denied that they had more than 4 children by themselves when the youngest child was an infant or a toddler. On 10/26/21, Staff member D denied that she had more than 4 children by herself with the youngest child being an infant or a toddler. On 10/26/21, I provided technical assistance to Ms. Murray stating that when the staff members are alone with the children, they need to ensure that they are providing care and supervision to the children at all times and not engage in any other activity that takes time away from the children. On 10/27/21, I checked the staff to children ratio for the weeks of 8/23/21 to 8/27/21 and 9/6/21 to 9/10/21. On all of these days, the classrooms maintained appropriate ratios. After I reviewed the attendance records, I provided technical assistance to Ms. Jamil and Ms. Murray reiterating the importance of maintaining accurate attendance records with accurate sign and sign out times for staff and students.

    On 9/20/21, the department received a complaint stating that Ms. Murray was not present at the center most of the time. Complainant B did not know the dates or times when Ms. Murray was not present at the center. On 9/27/21, Ms. Murray stated that she is present at the center at least 6 hours per day unless she has a doctor's appointments or other work for the center. She said that the center is open from 7:30 am to 5 pm or until the last child is picked up (maximum of 5:30 pm). Ms. Murray said that the enrollment has gone down since the pandemic and there are no children beyond 5 pm every day. On 10/18/21, I interviewed Staff member A, Staff member B, and Staff member C who stated that Ms. Murray was there at the center every day unless she has a doctor's appointment. On 10/26/21, I interviewed Staff member D who said that Ms. Murray is present in the center every day, but she was not sure about the hours. On 10/27/21, I reviewed the sign in sheet for Ms. Murray for the following weeks; 7/12/21 to 7/16/21 8/23/21 to 8/27/21 9/6/21 to 9/10/21 On all of the above-mentioned days, Ms. Murray was at the center for at least 6 hours every day.

    The center did not provide Child A's parents with a written daily record that includes the elimination patterns of Child A. Staff member A admitted that she did not record any entries for Child A's elimination patterns. All staff members indicated that they had not documented elimination patterns for Child B. There were no accurate entries made for Child C regarding diapering after 9 am on 9/27/21.

    The center combined the children of different age groups due to lack of enrollment. During the weeks of 8/23/21 and 9/6/21, the classroom maintained the required ratio. During my unannounced inspection on 9/27/21 and 10/26/21, the classroom was in ratio.
