    On 06/30/2023, I made an unannounced onsite inspection to the child care center from 10:18 am to 11:58 am. I interviewed program director, Melissa Fowlkes. Ms. Fowlkes denied the allegations that the child care center does not clean, sanitize, or disinfect surfaces and areas that sick children come in contact with. Ms. Fowlkes also denied that the child care center has or has had an outbreak of respiratory syncytial virus (RSV). Ms. Fowlkes stated that children who are or become ill with a diagnosed communicable disease cannot return to the child care center without a note from the child's physician. Ms. Fowlkes stated that sick children are not permitted to remain in care, and the center has a zero-tolerance policy regarding illness. Ms. Fowlkes shared that a child was recently sent home who had diarrhea, and the child cannot return to the center for at least 24 hours. Ms. Fowlkes also stated that children who may take medication for an illness, and the parents have not completed the medication forms the center requires, must have completed the medication dosage before they return to the center. Ms. Fowlkes stated that the child care center has contracted janitorial services with a local company that comes three times per week, to thoroughly clean the center. While the center has a janitorial service that cleans the child care center, Ms. Fowlkes stated that she and her staff clean the center every day. Child care staff members are responsible for cleaning and sanitizing their classrooms. Child care staff members clean their classrooms during naptime and throughout the day. Ms. Fowlkes cleans the public spaces/areas of the child care center such as the hallways, bathrooms, and the common area (large open space in the middle of the child care center). Ms. Fowlkes shared that the center uses the three-step process of wash, rinse, sanitize; diluted bleach and water is used as the sanitizing agent during child care hours. The child care center also uses Betco cleaning products and solutions to disinfect and/or sanitize areas and surfaces. Ms. Fowlkes shared that if children become ill during the day, or has symptoms of a communicable disease, the child is moved to the dedicated quarantine zone, the office to wait to be picked up by a parent or other approved person. When a child has a diagnosed illness or when a parent notifies the center of an illness their child has, the center posts a notice for all staff and families of the name of the illness, the symptoms, incubation, and communicable period, prevention, and suggested treatment. The notice is sent to all families through the center's communication app and is posted on the entrance/exit door, and the kiosk where families sign in and out. The child care staff members thoroughly clean their classrooms including sweeping, vacuuming, mopping, washing, and sanitizing toys, and children's linens are sent home to be laundered. Ms. Fowlkes shared that the last known diagnosis of a child with RSV or other respiratory illness was in October of 2022. When the parent 611 W. OTTAWA. P.O. BOX 30664. While onsite, I obtained copies of the center's illness policy, cleaning and sanitization policy, hand washing procedure, daily/weekly cleaning schedule, and health and safety exposure level procedures. On 06/39/2023, I interviewed child care staff member (CCSM) 1. CCSM 1 shared that if a child is sick or becomes sick during the day, the parent is called immediately, and the child is sent home. If a child becomes sick at home, the parent notifies the center, and a health bulletin is sent out. CCSM 1 shared that the center had an outbreak of a communicable disease in November of 2022.     On 2/23/2023, I called the complainant and left a message. I did not receive a return call. On 2/24/2023, I completed an onsite inspection. I spoke with program director, Melissa Fowlkes. She denied that infants are anywhere other than the infant room. She has staff scheduled to open each room when the center opens at 6:30 a.m. When infants arrive, there are staff ready to take them. Ms. Fowlkes also noted there is not a large room that any children gather in. I observed the infants only in the infant room. There was no infant equipment in any other area of the center. The lobby area would be the largest area in the center. It is not set up as a classroom. I did not observe any children in the lobby. On 3/22/2023, I spoke with Child Care Staff Member 1 (CCSM1). She stated that infants are only in the infant room. There are staff there from open to close and the infants never join with other classes. On 5/23/2023, I spoke with Child B's (male, 1 year) mother. Child B's mother stated she has been using the facility for 10 years and loves the school. She has only seen infants in the infant room.

    On 2/24/2023, Ms. Fowlkes stated the infants are never laid in bed with their bibs on them. I observed the infant room. None of the children lying in cribs had bibs on them. On 3/22/2023, CCSM1 confirmed that all staff know to take the bibs off babies before they go to sleep. She has not seen a baby with a bib on while sleeping. On 5/23/2023, Child B's mother stated that the staff in the infant room are excellent. The staff are in one room with the infants monitoring them all day, every day, and if any of them saw a baby with a bib on while sleeping, they would take it off immediately.

There is no room for car seat storage in the center. I did not see any car seats in the facility during my inspection. On 3/22/2023, CCSM1 stated she has not seen infants kept in car seats. The parents must take the car seats out of the center once they drop their child off. On 5/23/2023, Child B's mother stated she has not seen infants in car seats and there are no car seats in the building.

    On 2/24/2023, Ms. Fowlkes was adamant that all children are laid on their backs to sleep. As the children reach the milestones of rolling from back to tummy and tummy to back, they place stickers on the cribs notifying all staff. Only at that time will they let a child remain on their stomach is if he or she rolls from their back to get a more comfortable position. On 3/22/2023, CCSM1 confirmed that all children are placed on their backs when they are laid down to sleep. There were five children lying in cribs at the time of my onsite inspection. I checked the informational stickers with the children's milestones to make sure they were lying appropriately.
