On 05/17/2022, Ms. Walter emailed an incident report, and according to the incident report, at 11:00 a.m., on 05/17/2022, when returning inside from the playground Child A was briefly separated from her preschool classroom. Child A was returned to her class by the pre-k child care staff member. On 05/17/2022, I interviewed Ms. Walter at the center. Ms. Walter stated Child A's preschool classroom is normally in preschool room 1 but moved into preschool room 2 because the air conditioning is being repaired in preschool room 1. While outside 2 to go back inside, Child A lined up at the wrong door. She lined up at her normal preschool room 1's door. As Child A's class went inside the center, the pre-k class came outside to the playground. The pre-k lead caregiver saw Child A on the playground and took her back to the preschool classroom. The child care staff members complete face to name calls prior to going outside on the playground and when they return inside. When the face to name is being completed while the children are outside, there is always one child care staff member outside with the children and one child care staff member inside the classroom. I interviewed Ms. Leggett at the center on 05/17/2022. She said she is a child care staff member in the preschool classroom. When the children lined up against the wall on the playground, she was outside with the children and Ms. Montague was inside the classroom waiting for her to call the children's names so they could go inside the classroom. She called the children's names one at a time and they went inside the classroom after their name was called. She looked around the playground to make sure no child was left behind. She did not see any children, so she went into the classroom. She believed Child A went into the classroom because when she called her name a child walked into the classroom. Ms. Montague noticed that Child A was not in the classroom, and at the same time a pre-k child care staff member brought Child A back to the preschool classroom. Child A was on the playground alone for about one to two minutes. Ms. Leggett acknowledges that she made a mistake. She has been working at the center for about a month and she is still learning the children's faces. She used the paper form for the name to face call to check the children off when they went inside the classroom. There is also an iPad that can be used when completing the name to face call. The iPad lists the children's names, and it also has a picture of the children. I interviewed Ms. Montague at the center on 05/17/2022. She said she is the lead caregiver in the preschool classroom. Ms. Montague wanted to have Ms. Leggett practice putting the children's names to their faces since she was a newly hired child care staff member. Ms. Montague was inside the classroom while Ms. Leggett was outside with the children who were lined up against the wall waiting to go back inside the classroom. Ms. Leggett called the children's names, and they went inside one at a time. Once the children were in the classroom, Ms. Montague reviewed the face to name sheet and saw that Ms. Leggett circled Child A's name, but Child A was not in the classroom. At that time, Ms. Niemi brought Child A back to the preschool classroom. Child A was outside alone for about two minutes. I interviewed Ms. Niemi at the center on 05/17/2022. She said she is a child care staff member in the pre-k classroom. Ms. Niemi, Ms. Dunlap, and the pre-k class went outside on the playground at 11:02 a.m. Ms. Niemi heard crying by the preschool room 1 door. She went to the door and saw Child A crying. As Ms. Niemi was walking towards Ms. Dunlap with Child A, Ms. Montague opened the preschool room door. She then handed Child A to Ms. Montague. During all transitions, child care staff members say the children's names and look at their faces before they go into a room. 3 On 05/17/2022, I observed the fenced playground and there were no visible safety hazards. On 10/22/2021, I made contact with program director, Rebecca Walter. Ms. Walter self-reported concerns made by Child A's parent regarding inappropriate physical 2 contact by CCSM, Ms. McCleary. Ms. Walter shared she conducted her own investigation and showed the parent video footage and there were no findings. On 10/25/2021, I made contact with Child A's mother. She shared that last Wednesday (10/20/2021) while logged into the Watch Me Grow parent app she observed CCSM, Ms. McCleary being physically aggressive towards Child A. She shared she had a couple issues with Ms. McCleary that she's brought to the attention of Ms. Walter. She believes Ms. McCleary may not be too happy with her son because of stories she's shared with her about her and Child A's interactions. She shared there was one incident when Ms. McCleary informed her that during nap time Child A refused to sleep or sit quietly. When she told him to nap, he told her no. When he refused, she told him that he could help her clean the room and he used profanity towards her. Child A's mother said when she asked Child A about this, he didn't even know the word that Ms. McCleary said he used. This was her first time interacting with Ms. McCleary. She stated that there was another incident that made her uncomfortable during pick up. She walked into the classroom and noticed the children were having circle time. Ms. McCleary remained seated at circle time but yelled across the room to Child A and told him to tell her what he did. It was at this time that other children began to yell Child A was a "bad boy". She spoke with Ms. Walter about her concerns with how inappropriate Ms. McCleary handled the situation and, in the future, she would like to be addressed in a way that she felt didn't shame her child and was private. Ms. Walter told her she would speak with Ms. McCleary and use the incident as a teachable moment. Due to concerns she had with Child A's care, she started logging into the parent app to watch interactions between Child A, other children and the CCSM's. After a couple days of logging in she noticed Child A rarely participated in circle time. She would observe him sitting off to the side but rarely actively engaged or sitting in the actual group. When asked about this she was told that children are not forced to participate in circle time. She shared concerns that Child A is allowed to choose not to participate and frustrations that the CCSM's aren't able to manage "a 4-year-old's behavior and figure out a way to encourage him to participate." She spoke with Ms. Walter and expressed concerns that she wasn't made aware that Child A was refusing to participate in groups. She told Ms. Walter that she would speak with Child A, and moving forward she would like for him to participate and for him to not be given other options if he refuses to participate. She later had a meeting with Ms. Walter and the curriculum director at which time she was told Child A was having a difficult time listening, following directions and refusing naps. Child A's mother shared her expectations with the facility, which is not to provide Child A with other options when he refuses to participate. On October 7, 2021, during her contact with Ms. McCleary, she was told by Ms. McCleary that she told Child A he could not participate in group because he refused 3 to wear his face mask and even told Child A who had an ear infection that his infection was as a result of his mask refusal. Child A's mother shared she could not believe Ms. McCleary thought this was okay to share with Child A considering his ear infection had nothing to do with mask refusal. On October 20, 2021, while logged into the parent app (around nap time) she watched Ms. McCleary put sheets on the cots. She shared she could easily spot Child A because he had on a red sweater. The facility did not exclude child A from daily learning experiences. When the child refused to participate in circle time, an alternative activity was provided. When the child was asked to leave the group due to him swinging his face mask, which had been in his mouth, the child was supervised and provided an ooaortunity for an alternative educational option.
