Based on the interviews that were conducted and the documents that were reviewed, there is sufficient evidence to support that Lead Caregiver did not act in a manner that is conducive to the welfare of children. She was not truthful when asked if she had alcohol delivered to the child care center on 10/14/2024. She denied that she had alcohol delivered to the child care center on 10/14/2024, however, the delivery bag was labeled with her name, the date and time of the delivery, and the items that were delivered. It cannot be determined that Lead Caregiver consumed alcohol while at the child care center because Ms. Barry nor any other CCSMs saw Lead Caregiver consume alcohol while at the child care center. Lead Caregiver did not make an appropriate judgement and did not act in a way to promote the safety and well-being of the children when she had alcohol delivered to the child care center.

On 05/06/2022, Ms. Barry emailed an incident report, and according to the incident report, at about 11:30 a.m., on 05/05/2022, Child A was found with hand sanitizer in his hand. A small amount was underneath his mouth on his chin. There was no vomiting or diarrhea. Child A's father and poison control were called. Poison control advised the center to send Child A to the emergency room (ER) and give him food and water. Child A took sips of water and bites of a cracker. His father took him to the ER. Later that day, Child A's father reported that Child A was okay. 2 On 05/09/2022, I interviewed Ms. Barry and Ms. Smith at the center. Ms. Barry stated she was not present at the center when the incident occurred. Ms. Smith stated she was the only child care staff member present with four children in the infant/toddler classroom. During naptime on 05/05/2022 around 11:20 a.m., she was putting one of the children down for a nap. When she turned around, Child A was in the play area with the bottle of hand sanitizer in his hand. Hand sanitizer was around his mouth, on his clothes, and on the floor. The hand sanitizer was kept on top of a floating wall shelf. Ms. Smith was unsure how Child A was able to get ahold of the hand sanitizer because it was on the shelf which is too high for him to reach. She believes he may have bumped into the shelf causing the hand sanitizer to fall to the ground and then he picked it up. Ms. Smith called Child A's parents and was able to speak to his father. She also called poison control and they recommended that Child A be taken to the ER and given food and water. Child A ate crackers and drank water. He was not vomiting and showed no obvious signs of being sick. Child A's father picked him up from the center and took him to the hospital. Later that afternoon, Child A's father picked up his siblings from the center and informed the center that he was doing fine. Ms. Smith uses hand sanitizer when she cannot quickly wash her hands in between wiping the children's noses. Ms. Barry stated she removed the bottles of hand sanitizers from the two classrooms. She is thinking about installing hand sanitizer dispensers on the walls outside of the classrooms. On 05/09/2022, I observed the infant/toddler classroom. The classroom is divided into three sections; the eating area, the play area, and the sleeping area. The floating wall shelf is in the play area and is approximately 4 feet from the floor. On 05/25/2022, I interviewed Child A's mother by telephone. She said the center called and informed Child A's father that Child A had eaten hand sanitizer. Ms. Smith did not know how he got ahold of the hand sanitizer, and she did not provide any details about what was happening in the classroom when Child A got ahold of the hand sanitizer. The center called poison control and was informed that Child A should be taken to the ER. The parents took him to the ER where he was observed, then released without receiving any treatment. Child A's mother has no concerns with the center. Child A is doing fine.
