On November 1, 2022, I received an incident report from Childtime in Shelby. This incident report was a self-report regarding an incident that happened in the Early Preschool room on October 25, 2022. A message had been left by then Program Director Jennifer Buechel on October 26, 2022, fulfilling the 24- hour licensing notification requirement. *Thee ort detailed that on October 25, 2022, Child Care Staff Member (CCSM) scratched Child A on the back of his neck while trying to keep him on t during naptime. Child A allegedly tried to get off the cot and moved to do so. sitting next to him, reached for him and scratched the back of his neck. We immediately applied ice to the area and informed administration of what had occurred. Using the information on the incident report I contacted Child A's Mother on November 3, 2022. Child A's Mother stated that on October 25, 2022, she received a call from Assistant Director Cara English. Ms. English told her that during nap time Child A was trying to roll off the cot (3 inches off the ground). To prevent him from rolling off reached to pull him back on and in the process scratched his neck. Chi s Mother was not told during that call the severity of the scratch. She received an incident report from the school when she arrived to pick Child A up for the day. A few hours after the alleged incident. What she was told and what was written on the incident report she received were not the same explanation. She also looked at the back of Child A's neck and was astonished to see how bad it looked. She feels Ms. English downplayed what had happened. Child A's Mother took a photo of the back of Child A's neck when she got into the car. (Photo in file) She called back later and requested to see the video surveillance of what happened. Child A's Mother and Child A's Father went on a later day to review the video material. What they watched was what Area Director April Cole said was when the incident took place. Child A's parents do not believe after reviewing 3 the footage several times that was when the incident took place. There was no other footage from that day saved for them to observe. Due to the injury, the explanation does not make sense to Child A's Mother of when it happened. She agrees that Child A has difficulty napping and moves around quite a bit. She does not disbelieve that he wanted to get off a cot, but in the footage he does not appear distressed at all after the alleged scratch. stated that she would forward me an email including both the picture and the incident report she received. After a review of the email, I found that the incident report did detail the roll off the U cot but also included that Child A had been running around all day and from -. Child A's Mother wanted to know why that statement about Child A running around all day had anything to do with the incident itself. The picture shows an approximately 4inch long linear scratch starting top left trailing down to the right. In the middle of the scratch the skin was broken. The area around the scratch was red and swelling. Child A's Mother did not seek medical attention for the scratch but is using a cream to prevent infection. On November 15, 2022, I conducted an unannounced inspection at Childtime. Upon arrival I met with Assistant Director Cara English. Ms. English has worked at the center for 7 years in various capacities. Ms. English stated that she was at the center when the incident occurred. came to the office and informed Ms. English what happened. did not down play how bad the scratch looked to Ms. English. Ms. English did not go to see the scratch before calling Child A's Mother. Ms. English admitted that she never even saw the scratch that day. When Child A's Mother came to pick up Child A that day the two spoke mostly about Child A's behavior on that day.
