    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 11/29/2022 and incident report was received from Sault Cooperative Preschool. On 11/22/2022 Sault Cooperative Preschool received homemade cupcakes from a parent. Child A was served a cupcake at snack time. It was unknown at the time that the cupcakes were decorated with Reese's peanut butter candies. Child care staff member Debra Campbell was sitting at Child A's table during snack and did not observe Child A eat any part of the cupcake. After the children were done eating, Ms. Campbell ate one of the cupcakes and realized that the cupcakes were decorated with Reese's peanut better candies. Ms. Campbell was not concerned; she did not believe Child A had eaten any part of the cupcake. About 30 minutes later Child A vomited. Staff believed it was from a previous illness that had kept Child A home the previous week. Staff called Child A's mother and Child A's mother picked her up. That evening Child A's mother contacted Sault Cooperative Preschool stating that Child A had an allergic reaction. Child A's mother reported that her face broke out with hives and her eyes swelled shut. Child A told her mother that a funny skittle made her mouth feel funny at school. On 12/07/2022 I completed an on-site inspection at Sault Cooperative Preschool. I interviewed program director and licensee designee Stacie Piontek. There are three children this year with food allergies. The children's allergies are written on their name cards located at their seat as well as in the classroom on the snack sheet. She did not find out about Child A being fed peanut butter until Child A's mother contacted her on 11/23/2022. Child A's mother also reported that Child A was also given yogurt by a staff member previously, which is also listed as an allergy for Child A. Child care staff members Bonnie White and Debra Campbell are the staff assigned to Child A's classroom. On 12/07/2022 I reviewed the child information record for Child A. The allergies section listed: allergies- egg, dairy and peanuts. I also verified that Child A's allergies were listed on her name card and the snack sheet. 3 On 12/15/2022 I interviewed Child A's mother via telephone. Child A attended Sault Cooperative Preschool for about 2.5 months. Previously Child A was fed yogurt by the preschool; she contacted the school and they apologized. Child A did not have an allergic reaction to the yogurt. On 11/22/2022 the school called her and reported that Child A threw up. Child A had been sick; Child A was pale when she picked her up. When they got home Child A reported she ate a weird skittle and it made her mouth feel funny. Child A's eyes and face swelled and she developed hives. The school told her that she didn't eat anything that day. The school sent her a picture of Child A with the cupcake decorated with Reese's peanut better candies sitting in front of her. Child A will not be returning to Sault Cooperative Preschool. On 12/15/2022 and 12/21/2022 I left messages for Child B's mother; Child B also had allergies. As of the date of this report I have not received a return phone call. On 12/15/2022 I interviewed Child C's mother via telephone. Child C (female, 4 years) has a dairy restriction. She mostly packs Child C's food. She is not aware of Child C ever being fed dairy at Sault Cooperative Preschool. She has no concerns with the care or supervision Child C received at the preschool; they are really helpful. On 12/15/2022 I interviewed child care staff members Rachel Pratt and Haley Graves via telephone Neither of them was in Child A's classroom when the incident occurred on 11/22/2022. They were both able to tell me where the allergies are listed for the child care children. They reported that they were not aware of any previous issues with children ingesting something they were allergic to. If a child did ingest something they were allergic to they would call the parents rights away. There are no concerns with the care and supervision children receive at Sault Cooperative Preschool.
