    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times. 4

    On 04/08/2021, I completed an unannounced on-site inspection at Mustang Hill Daycare. I met with licensee designee/program director Julie Sheets. Ms. Sheets was not present at the center at the time of injury. The incident occurred on 04/05/2021 at approximately 2:30 p.m. and the injury was reported to the department via telephone on 04/06/2021 and a written incident report was submitted to the department on 04/06/2021. The lead caregiver Diane Lanphierd was supervising Child A (male, age 7) on the playground at the time of the injury. Ms. Lanphierd immediately provided first aide to Child A and brought him and all the children inside with the assistance of other staff. Ms. Lanphierd applied pressure with paper towels, applied an ice pack and provided comfort to Child A until his aunt and mother arrived. Another caregiver contacted Child A's mother and his aunt who is listed as an emergency contact on the child information card. Child A's aunt 2 arrived within 10 minutes of the injury. Child A's mother arrived 20 minutes after the aunt arrived and she drove him to Mid-Michigan Medical Center — Midland emergency department. On 04/05/2021, Child A's mother reported that he required nine staples to the back of his head. Child A received medical treatment in the emergency room and was discharged the same evening. Child A has since returned to the center and remains in care. I then met with and interviewed secretary Candi Schoenherr. She did not witness the injury however was inside the center and saw on the video cameras staff and children running at the same time in the direction of the merry-go-round and she figured something had happened and decided to go outside to the playground. When she got outside Ms. Lanphierd was providing first aid to Child A by applying pressure to the back of his head. She assisted in getting all the other children inside while Ms. Lanphierd walked Child A inside. Once inside Ms. Lanphierd applied pressure with paper towels, applied an ice pack to the injury and provided comfort to Child A. Another caregiver contacted Child A's mother and his aunt as she lives nearby and is listed as an emergency contact. Child A's aunt arrived within 10 minutes and waited for Child A's mother to arrive. Child A's mother drove him to the emergency room to receive medical attention. Staff did not contact 911 as Child A did not lose consciousness, the bleeding was controlled, and he did not appear to be in distress. I then obtained a copy of the Playground Inspection Certification Summary dated 7/13/2011 in which the Merry-Go-Round is listed as approved for ages 5 — 12 years. I also obtained a copy of Child A's child information card. On 04/08/2021, I attempted to contact Child A's mother by telephone and there was no answer, and I was unable to leave a voice mail message. On 05/27/2021, I contacted Child A's mother by telephone and an interview was conducted with her and Child A's father via speaker phone. Staff contacted her and her sister by telephone after the injury occurred. Her sister arrived at the center within 10 minutes of the injury occurring as she lives nearby. She arrived at the center within 30 minutes of the injury occurring as she works in Saginaw, MI. Child A was driven to Mid-Michigan Medical Center — Midland Emergency Room where he received nine staples to the back of his head and was discharged home the same evening. Child A did not sustain a concussion or any other injuries. The cut was 3 inches long and 4 cm deep. Child A has recovered well and returned to child care the following week. Prior to this injury the parents never had any concerns with the quality of care provided by the staff. Initially they had some concerns that the Merry- Go-Round should not be approved for children's use. They spoke to the school superintendent and requested he determine if it should be removed. (11) The playground equipment, use zones, and surfacing in the outdoor play area must be inspected by a certified playground safety inspector and an approval granted for playground equipment and areas before issuance of an original license, upon request of the department, and before using any newly added playground equipment. The center shall provide documentation of the inspection to the department upon request and shall keep it on file at the center.

    The center had Michael A. Kozuch from KJP Sales, Inc. (NP SI) complete a playground inspection on 07/13/2011 before the playground equipment was used by children in care. The merry- go-round is approved for ages 5— 12 per the Playground Inspection Certification Summary report dated 7/13/2011 which was provided to the department and is kept on file at the center.
