On October 28, 2022, licensing consultant Stacy Wables sent an email to complainant requesting clarification regarding allegations. An email has not 2 been received to date. On October 30, 2022, I made a telephone call attempt to complainant. I left a message requesting a return telephone call regarding clarification of allegations. A return telephone has not been received to date. On November 2, 2022, I completed an unannounced on-site inspection at the child care center. I individually program director Cassandra Wright, child care staff members, Megan Mata, Yvette Murray and Jewell Perkey. Play and Learn Children's Place rents space from the Evangelical Homes of Michigan and shares the building with their skilled nursing facility. The nursing facility is located on the north side of the building and the child care center is located on the south side of the building. Program director Cassandra Wright stated that October 20, 2022, there was a malfunction with the fire alarm system on the nursing facility side of the building, which caused the fire alarm to activate. Ms. Wright stated the child care center evacuated the building when the alarm was heard. The preschool age children were in the lunchroom when evacuation was required. They exited through a door in the lunchroom, which leads directly outdoors to an enclosed play area. This is an enclosed area with a locked gate, as this area leads to the parking lot and sidewalk. Ms. Wright stated that two days prior the gate was not properly closing, and she secured it closed with zip ties, and scheduled to have the gate replaced on October 24, 2022. Ms. Wright stated that she took the zip ties off the gate during the evacuation, as she observed child care staff members having difficulty opening the gate. A new gate was installed on October 24, 2022. Infants and toddlers were in their rooms during the time of evacuation. They exited from a different door closest to their classrooms, which did not require them to pass through the gate. Child care staff members Megan Mata, Yvette Murray and Jewell Perkey all stated they had difficulty opening the gate on October 20, 2022, during the evacuation. The gate was secured close with zip ties, and they were not able to unfasten the zip ties until assistance was provided. I observed the new gate to have a lock that was accessible by adults with no assistance. The gate was observed to have no zip ties securing it closed. I provided consultation and technical assistance on properly maintaining unobstructed means of egress.
