I completed an unannounced, on-site inspection on 06/17/2022 and spoke with Ms. Bruzdowski regarding all four of the allegations in the complaint. I shared with her the allegation that child care children have been observed playing in the middle of the road and that she has not been supervising them. Ms. Bruzdowski stated that is "completely false." The children mainly play in the fenced in backyard, however there were a few occasions when the children were playing in the driveway or sidewalk because they were using chalk. Ms. Bruzdowski reported to doing this activity "maybe three times." When the children were playing with the sidewalk chalk, Ms. Bruzdowski was sitting right near them. There is a tree line to the driveway that she would allow the children to draw with the chalk. Ms. Bruzdowski would do this only with the children she could trust to stay within those boundaries. Ms. Bruzdowski's house is on a corner lot and the garage/side walk area is on the side. Ms. Bruzdowski indicated on occasion she will sit on her front porch with the children and wait for their parents to arrive. On 07/06/2022, I spoke with the complainant who stated that they had not personally witnessed any of the allegations made against Ms. Bruzdowski. However, they had heard the information from different individuals. The complainant was not sure why the individuals had not filed their own complaint but felt they should file a complaint so that the department has been made aware. I spoke with Child A's (4-year-old, female) Mother on 08/08/2022. Child A has been attending child care with Ms. Bruzdowski since July 2021 and averages 2-3 days a week. Child A's Mother reported that there has never had an issue with care and supervision. Ms. Bruzdowski has "done nothing but good and Child A's well-being has never been in question in her care." I spoke with Child B's (3-year-old, male) Guardian on 08/08/2022. Child B has been attending child care with Ms. Bruzdowski since May 2022. Child B attends full time, 5 days a week, and there are no concerns with the care he receives from Ms. Bruzdowski. Child B has responded well to the schedule and routine in Ms. Bruzdowski's home. There are no concerns with the animals in her home or supervision. Ms. Bruzdowski utilizes a Pro Care application to enter what Child B eats and does throughout the day. Ms. Bruzdowski will also send pictures of Child B's activities throughout the day.
Children have been observed playing in the middle of the road. Ms. Bruzdowski is outside with the children, but not supervising Ms. Bruzdowski has two big dogs and multiple cats. A child was seen with scratches, although it is unknow where the child was scratched or if the scratches were due to the animals. Ms. Bruzdowski placed the children in her car to go shopping and they were not in proper safety seats. Ms. Bruzdowski is not supervising the children when they go to the bathroom. A little boy was found looking under the stall of a little girl and made a comment about her private area. It is unknown when this occurred.

During the 06/17/2022 on-site inspection, I observed the Ms. Bruzdowski's two dogs. The first is a Boxer that is mixed with either American Bull Dog or Pit, the family is not certain, named Tator Tot. The second dog is a Berne doodle, named Coco. Ms. Bruzdowski admitted that Coco ended up being a larger dog than they imagined. Ms. Bruzdowski has one cat, named Harley. Ms. Bruzdowski admitted that the dogs are "door aggressive" and will bark when the doorbell is rung or someone comes into the home. She will place the dogs behind the gate in the kitchen/living room area, or in a bedroom, or outside when someone enters the home. It has been these consultants experience that Ms. Bruzdowski puts the dogs outside when I have been to her home. Ms. Bruzdowski described the dogs to be good with the children. Ms. Bruzdowski does allow the dogs to interact with the children and reports she has not had a reason not to. She reports to separating the dogs from the children when they are eating and this consultant observed their food and water bowls to not be accessible to child care children during the on-site inspection. Regarding child receiving scratches from the pets, Ms. Bruzdowski has not witnessed any incidents of this, nor have any parents mentioned anything concerning scratches to her. Ms. Bruzdowski reported that she has seven children currently enrolled in her child care. I reviewed the child in care statement and parent handbook. Both documents indicate that there are pets in the home. A review of the documents confirmed that child care parents had signed the child in care statement, as well as a receipt of receiving the parent handbook. When I spoke to Child A's Mother and Child B's Guardian on 08/08/2022, neither had expressed concerns regarding the pets or the children receiving unexplained scratches.

During the 06/17/2022 on-site inspection, Ms. Bruzdowski confirmed that she had obtained permission from child care parents for routine transportation to take her son to and from school. I reviewed seven children's records that documented parents granting routine permission. Ms. Bruzdowski also had documented parental permission to take child care children on walking field trips around the block, to nearby parks, and local businesses. Ms. Bruzdowski admitted that on a few occasions, they would stop at a local park to play that they passed on the way home from school. However, she denied ever doing random or unapproved trips to the store or to go shopping with child care children. Ms. Bruzdowski is aware of the need to have the children properly restrained and stated that she had always done so. Ms. Bruzdowski currently has four car seats and two that are front facing and two that are rear facing.

During the 06/17/2022 on-site inspection, I reviewed this allegation with Ms. Bruzdowski. She immediately knew the situation and referenced an incident report that occurred on 05/02/2022 involving Child A and Child C (4-year- old, male). Ms. Bruzdowski had submitted an incident report to the department. The situation involved a Doc McStuffins toilet training chair. Ms. Bruzdowski had just come out of the bathroom from changing another child's diaper and observed Child A (while clothed) sitting on the toilet training chair and then heard Child C make a comment about her privates. The children were in the living room. Ms. Bruzdowski contacted both Child A's Mother and Child C's Father. I spoke with Child A's Mother who confirmed that Ms. Bruzdowski had discussed the incident with her and that she was contacted immediately and felt comfortable with how she handled the situation. Ms. Bruzdowski disenrolled Child C from her child care on 05/02/2022.
