On March 3, 2023, I received allegations that Child Care Staff Member 1 (CCSM) did not stop an outdoor playground altercation or tell the parents the incident involving Child A could have resulted in injury. The Complainant provided Child A's Mother's contact information. I interviewed Child A's Mother over the telephone. Child A's Mother stated that on March 2, 2023, she picked up Child A from care at the end of the day. While she was in the building no one told her that there had been any incidents with Child A that day. She received no report, verbally or written, that Child A could have possibly been hurt. While she was putting Child A into the car Child A told her that Child B had stepped on his genitals while playing on the playground. He told his mother that his teachers knew it had happened. He appeared to still be in pain and upset over the incident. She went back into the childcare, found CCSM 1 and asked about the incident. CCSM 1 concurred that it happened but stated that she had not had the time to complete an incident report before Child A had been picked up. When Child A's Mother asked why CCSM 1 had not verbally told her CCSM 1 stated it was because it was his "privates." This is not the first time that Child A's Mother has not been told about an incident involving injury. She does not want this to become a pattern with CCSMs not telling her or other parents that something happened causing injury. This time Child A told her no one checked him to see if he was hurt. No one told her so she could check to see if he sustained any injury. On March 7, 2023, I conducted an unannounced inspection at Lakeshore Learning Center. Upon arrival I met with Owner/ Licensee Designee Pat Butz. Ms. Butz was aware of the situation. She stated that she did not see the incident on the playground between Child A, Child B and Child C (4-year-old female) but was informed by CCSM 1 what happened. According to Ms. Butz, CCSM 1 did not complete an incident report before Child A's Mother arrived at the center. Ms. Butz knew at the time of occurrence that it was Child A's genital area that was hurt. She did not check this area for injury and, to her knowledge, CCSM 1 did not check. She agrees that Child A's Mother should have been made verbally aware at the time of pick-up. She believes that CCSM 1 was embarrassed to say that the incident included Child A's genitals. I then interviewed CCSM 1. She stated that she is a Lead caregiver in the preschool room. She normally has a group of 10 to 12 children and CCSM 2. On March 2, 611 W. OTTAWA. P.O. BOX 30664. She was about 20 to 30 feet away from Child A, Child B, and Child C with another group of children. She saw Child A on the ground. Within seconds she saw Child B step on Child A's genital area. CCSM 1 was already walking towards the group of children when the incident happened. She separated them and checked on Child A. She knew he had been hurt by his facial reaction when it happened but did not check due to it being his "private area." It was only about an hour and a half between the time of the incident and Child A's Mother picking him up. She did not have time to complete an incident report. As of the inspection date and time a report had still not been completed. I asked her why she had not verbally told Child A's Mother what happened that day and she stated it was because it included his genital area, and she was uncomfortable discussing it. I then asked what the incident/accident reporting policy was regarding letting parents know about those events. CCSM 1 stated that parents were informed immediately for all emergencies and same day for accidents/incidents regarding injury. We agreed that because CCSM 1 did not check Child A's genitals Child A's Mother should have been informed same day. After the interview, I spoke again with Ms. Butz and was
Child A (4-year-old male) was stepped on by Child B (4-year- old male) while outside playing on the genital area possibly causing injury. Child A's Mother was not notified or provided with an incident report at pick up.
