R 400.1907 · R 400.1907 Child's records. (3) Dated daily attendance records of children in care must be maintained and include the child's first and last name and the time of arrival and departure. Electronic records may be used. If electronic records are used, they must be available to the department at the time of the inspection. If electronic attendance records are not available during an on-site inspection, the child care home is in violation of this rule.
On 7/14/22 and 8/08/22, I interviewed Child A's Mother. Child A attended Ms. Prenzler's child care home on Monday and Tuesday 7/11 and 7/12/2022. She had been off for three weeks prior to that. On Monday she attended from 10:00 a.m. until about 6:00 p.m. and Tuesday she attended from about 7:00 a.m. until 5:00 p.m. On Tuesday, 7/12/22, Child A's Mother picked up Child A from child care. She went downstairs and Ms. Prenzler and Child A were at the bottom of the stairs. Ms. Prenzler told Child A's Mother that she was coloring on herself and she had a rash and hives on her. Child A's Mother told Ms. Prenzler if she thought Child A was having an allergic reaction that she should have contacted her. When they got outside, she could see that Child A's skin was rubbed off and it was bruised from excessive rubbing to get the marker off her. Child A told her that she said ow and cried while Ms. Prenzler was trying to get the marker off her. Child A's Father was upset and contacted Ms. Prenzler to ask her what happened to Child A's face. Ms. Prenzler said that she had to go to the bathroom. While she was in the bathroom Child A colored on herself with markers and Child A also doused herself with sanitizer. Ms. Prenzler attempted to clean the marker from Child A with a wash cloth. She did not notice a mark on her after she washed her face with the wash cloth. On 7/15/22, I made an on-site inspection with Tania Allard, child care consultant. Ms. Prenzler invited us into her home and we went to the basement. She took us into a room with two children. I could hear other children. I asked Ms. Prenzler if there were other children in her care. She told me yes and said they were playing in the other room. I asked if she could show me. She opened the door to a room where there were six children running around playing. There were a lot of storage items in the room. The children said they were playing circus. I requested that the children come out of the room because it was not an approved space for child care children. Ms. Prenzler explained what happened to Child A the other day. Ms. Prenzler had to go to the bathroom and Child A got into markers and wrote all over her face and arms. She and Child B (male, 4 years) got into sanitizer that was in a child's diaper bag. She washed the marker off with a washcloth. Child A said her face was burning but when she looked at Child A's face, she did not see anything wrong with it. Ms. Prenzler got a cold wash cloth and put it on her face. She still had marker on her face when Child A's mother picked her up. Ms. Prenzler stated that this was not the first time Child A wrote on herself with marker. Child A got into sanitizer the same day. She does not keep hand sanitizer accessible to the children. The hand sanitizer was in a child's diaper bag. Child A and Child B went into the bag and got out the hand sanitizer and opened it. CONCLUSION: I VIOLATION ESTABLISHED INVESTIGATION: On 7/15/22, Ms. Prenzler was caring for eight children. Upon review of her attendance record for the day there were five children that were on the record to be there at the time of the inspection. There were three children in care that had an arrival time of 3:30 p.m. Child C was present but not listed on the attendance record. Ms. Prenzler provided me with a copy of the attendance for the current week. I reviewed the record for the day. The attendance for the day of the inspection was not accurate. I reviewed the attendance record for Monday and Tuesday 07/11 and 07/12/22. These are the two days Child A was at the child care home.
