I observed the ratio requirements to be posted in various places around the child care center. The parent interviewed by telephone did not have any concerns with ratio. Upon looking through the refrigerator and upper cabinets a variety of food was observed to be out of date. Specifically, I observed one can of peach pie filling that expired in 2022, three cans of pumpkin pie filling that expired in May of 2024, one can of sloppy joe sauce that expired in November of 2023, and one box of Pasta Roni that expired in September of 2024. There was one half gallon of skim milk that expired three days prior on 3/10/2025.

I conducted an unannounced onsite inspection on 1/9/2025, and the child care staff members were not serving food that was not in sound condition, spoiled, contaminated, or unsafe for human consumption. I viewed all of the food in the cabinets, refrigerator and freezer and there were no i tems that were out of date or spoiled.

On 10/06/2020, I had email correspondence with the complainant source to gather additional information. The complainant source provided Child A's mother's contact information. On 10/06/2020, I completed an unannounced on-site inspection at Underground Railroad. I met with licensee designee, Amanda Johnson. Ms. Johnson is not aware of Child A (female, age three) or any other child getting a piece of metal stuck in their foot from a piece of the floor that was lifting. Ms. Johnson further explained that there is not any metal on the floors in the child care center. Ms. Johnson denied that the metal vents in the center are rusted. Ms. Johnson denied that the kitchen is dirty or that they serve expired food. Ms. Johnson reports that they purchase milk for the domestic violence shelter and the center and freeze it immediately in a subzero freezer. They take daily readings of the freezer temperature and keep a daily log, as required by the health department. When the milk is removed from the freezer, they date the milk for seven days out, as required by the department. Milk is discarded on the seventh day. I then interviewed program director, Mackenzie Richard. Ms. Richard is not aware of Child A or any other child getting a piece of metal stuck in their foot from a piece of the floor that was lifting. Ms. Richard denied that the metal vents in the child care center are rusted. Ms. Richard denied that the kitchen is dirty or that they serve expired food. Ms. Richard reports that they purchase milk for the domestic violence shelter and the center and freeze it immediately in a subzero freezer. They take daily readings of the freezer temperature and keep a log. When the milk is removed from the freezer, they date the milk for seven days out, as required by the department. Milk is discarded on the seventh day. I then completed a walk-through inspection. There are no metal pieces on the floor that are lifting in the child care center which is located in the lower level of the building. In fact, the floor has no metal at all in the center. The metal vents are painted and are not rusted. The food in the refrigerator was not expired. The milk was not expired either. Ms. Johnson and Ms. Richard denied that they were aware of anyone housed in the domestic violence shelter that is located on the upper level who had complained of any metal piece from the flooring injuring Child A or any other child. They both 3 agreed to check with the domestic violence shelter staff and the other two caregivers to ensure that the injury did not occur in the shelter. On 10/06/2020, I received an email from licensee designee, Ms. Johnson indicating that she had talked to all the caregivers, shelter staff and residential coordinators and none of them have any knowledge of a child hurting their foot in the entire building. Ms. Johnson further checked all incident reports and there was no report regarding a child hurting their foot. On 10/06/2020 and 10/29/2020, I attempted to contact Child A's mother by telephone and there was no answer. I was unable to leave a voice mail message, as the voice mail was full. On 11/10/2020, I contacted Child A's mother by telephone and an interview was conducted. Child A was not enrolled in care at the center and the injury occurred in the domestic violence shelter. The injury occurred in the playroom of the domestic violence shelter located on the upper level. She did not report the injury to staff as the injury did not require medical attention. The staff did inform her that the milk is frozen immediately when purchased and taken out of the freezer and dated for seven days and on the seventh day the milk is discarded. She had concerns with food being outdated in the shelter and still being served. Child A's mother has no complaints regarding the child care center as her child did not attend the center. On 11/18/2020, I attempted to contact the licensee designee Ms. Johnson via telephone and there was no answer. A voice mail mess
