On 01/09/2023 from 4:00 pm — 7:30 pm and on 01/12/2023 from 4:00 pm — 10:30 pm, Ms. Harris was caring for nine unrelated children by herself. Ms. Harris's family child care home is licensed for not more than six children. Ms. Harris was over capacity on 01/09/2023 and 01/12/2023.

R 400.1910 · p R 400.1910 Ratio of ersonnel to children. (1) The ratio of personnel to children present in the home at any 1 time must be not less than 1 member of the personnel to 6 children. The ratio must include all children in care who are not related to any personnel and any of the following children who are less than 6 years of age: (a) Children of the licensee. (b) Children of a child care staff member or child care assistant. (c) Children related to any member of the household by blood. marriaae. or adoption.

On 01/09/2023 from 4:00 pm — 7:30 pm and on 01/12/2023 from 4:00 pm — 10:30 pm, Ms. Harris was caring for nine unrelated children by herself. Ms. Harris is aware that the ratio is to be maintained at not less than one member of personnel to six children. On 01/09/2023 and 01/12/2023, Ms. Harris was out of ratio.

R 400.1951 · R 400.1951 Transportation. (7) Each child transported shall remain seated and properly restrained by a child passenger restraint device appropriate for his or her age. The manufacturer's rated seating capacity for the vehicle must not be exceeded. A

On 04/04/2023, I observed Ms. Harris transporting her two children and three children in care in her vehicle. The vehicle's manufacturer's rated seating capacity for the vehicle is five and Ms. Harris exceeded that by one. One of the children could not be properly restrained due to Ms. Harris exceeding the vehicle capacity.

R 400.1952 · R 400.1952 Child transportation; parent permission; child information card; required when off-premises. (1) A licensee shall obtain and keep on file written permission from a child's parent before a child is transported in a vehicle. Written permission must be obtained for both of the following: (a) Routine transportation, at least annually.

On 04/04/2023, Ms. Harris was transporting Child A, Child B and Child C without written permission from the child's parent on file, as required.

(3) A licensee shall have a copy of each child's information card and a first aid kit, containing the items listed in R 400.1951(9), accessible at all times when children leave the premises.

On 04/04/2023, Ms. Harris was transporting Child A, Child B and Child C without having a copy of their child information card in the vehicle when leaving the premises, as required.
