On 03/25/2021 during my on-site visit, I requested to review the records of Ms. Rohelia's child care staff members. There was no statement signed by a licensed physician or his designee that attests to the individual's mental and physical health, for the child care staff members, Chaka McMillan and Shampane Boutte. Ms. Rohelia did not have them and further stated neither worked there. Neither staff member had a certificate in infant, child and adult CPR or first aid. There was no verification that neither child care staff member had training in the prevention of infectious disease, including immunizations, or that Ms. McMillian or Ms. Boutte were fingerprinted.

The email contained pictures of child care rooms inside Ms. Rohelia's home as well as a blank signed Incident Report (BCAL 4605) and the attendance for the week of 02/15/2021. The BCAL 4605 was signed by Ms. Boutte. There were pictures of the living room, stairway upstairs and upstairs bedroom. The living room pictures showed the front exit door being blocked with a desk and two chairs. This exit door is an entrance/exit for childcare use that leads to the front yard. There were also two cabinets in the living room where approximately ten bottles of liquor on top of each cabinet. These bottles were accessible to the older children in care and could have been a hazard to the younger children if they fell. The pictures of attendance showed Child A and Child B marked present at 7:45am on 02/19/2021. 4 On 02/24/2021, I sent an email to , to let her know I was was the assigned licensing consultant. On 02/26/2021, I spoke with Shampane Boutte, child care staff member. Ms. Boutte states she was helping Ms. Rohelia out because she wanted to open her own child care home. She took orientation and had been coming over to Ms. Rohelia's home to assist since the beginning to obtain experience. On 02119/2021, Ms. Rohelia called her around 8:00aomfMans e told her she was on her way. Ms. Rohelia told Ms. Boutte she was just calling to see how she was doing. On this day, she picked up Child C years) on her way to Ms. Rohelia's child care home. She also ha I 0 ) with her. She does not , recall the exact time, but they probably arrived at Ms. home between 8:15- 8:17am. Child C went upstairs and she remained downstairs. She had a phone appointment with her at 9:00am which lasted 40 minutes. During this time, Child A did not stop crying. Child C brought Child A downstairs and continued to cry no matter what they did She attempted to give Child A peach oatmeal, fore. change and attempted a breathing treatment; however, none of these things soothe ild A and in fact, was crying harder. They called Child A's mother to see if something was wrong and she said nothing was wron when she dropped 0 off and was normal; but perhaps mmiisssseedd her because had not spent a 10 0 time wi er due to her schedule. i d A's mother stated s e would pick ■ up around 11:15 am. Child A's mother called back to indicate had some type of boil on the back of head and if she knew what ha ened. a told her no she did not ■ because although she held Child A, the side of head was laying on her and she did not feel any bump. Also, the ways hair was combed, she did not notice anything. Ms. Boutte states as far as she knew, Child A did not have any falls within the past several days, even though is trying to walk. Child B is Child A's and likes to play "momma" with but she was not there that morning. She was surprised when Ms. Rohelia o d her Child A's mother called from the to report Child A had a . Overall, Ms. Rohelia is awesome and she does not know how Child A got hurt. On 02/26/2021, 1 left a phone message for Ms. Rohelia. I also sent an email to her requesting copies of her attendance and child information records. On 02/26/2021, 1 received a phone message from Ms. Rohelia stating she was returning my call. On 03/0112021, 1 spoke to Ms. Rohelia. She said on 02119/2021, she had six 0 children. Child A was dropped off at 7:45am she thought but for sure by 8:00am. She denies failing or getting hurt in any way during the time was there. ■ left around 1:15am. Child A did fall on 01128/2021 whenl fel out of. booster E seat and hit face on the floor, also bumped forehead but she let mother know, he did not write ou a report for tha tneident because. ha no injuries.
