On 5/13/21, I interviewed Child A's (female, 22 months) Mother. Child A's Mother explained that they thought someone tried to pick-up Child A that was not supposed to but it was a misunderstanding. Tunisa Smith had called her to tell her that some woman was there to pick-up Child A and they got Child A ready for the woman. When they went to take Child A to the woman she was gone. Child A's Mother knew that her mom was going to pick up Child A and she would not leave without Child A so it was not her mother that was at the center. At this time, she believed that a random person was trying to take Child A and contacted the police. They discovered after reviewing the security tapes that the woman came to pick up another child from a different room, but none of the staff realized that the woman took a child from the building. There were two staff in the room at the time Tunisa Smith and Aviyon Beverly. Ms. Beverly is not working there any longer. Child A's Mother said that her mother has never been asked for identification when she has picked up Child A. Child A's Aunt has picked her up from child care before and she is not on the child information card as an authorized person for Child A to be released to. On 5/13/21 I went to the center. I interviewed Jenny Ogg, program director. Ms. Ogg has recently become the program director. She is aware of the situation that occurred with Child A. Tunisa Smith and Aviyon Beverly were the staff in the infant/toddler room. The woman that came to pick-up Child B asked for Child B but had a mask on and it sounded like the nickname that Child A is called and that is why Ms. Beverly was getting Child A. Ms. Ogg took me to the infant/toddler room. Shenikah Smith, lead teacher, explained that she was not working when the incident happened with Child A. She explained the pick-up process. Due to covid they are taking the children to the door when parents arrive to pick-up children. They have the child information cards that they can reference for the individuals that they are not familiar with. Ms. Smith provided me with the Child A's child information card. She was telling me that she is familiar with Child A's family and who picks her up from child care. She confirmed that Child 3 A's Aunt does pick Child A up at times. I reviewed the child information card. Child A's Aunt was not listed as an authorized person to release Child A to. I interviewed Tunisa Smith. Ms. Smith explained that she and Ms. Beverly were working. The doorbell rang and Ms. Beverly was holding a baby. She gave the baby to Ms. Smith and went to open the door. Ms. Beverly thought she asked for Child A. Ms. Beverly got Child A ready to go and when she went back to the door with Child A the woman was gone. Ms. Smith got Child A's emergency card and called Child A's mother to tell her what happened. She described the woman to Child A's Mother and she did not know her. The woman was gone because she had picked up Child B from the other classroom whose name sounds like Child A's name. Ms. Ogg stated that the police were contacted by Child A's Mother because she thought that someone had tried to take Child A. Ms. Ogg and the police were attempting to figure out what happened. Ms. Beverly would not cooperate. She no longer is working at the center because of this. When Ms. Ogg was reviewing security footage with the police officer, they pieced together what happened. The footage showed the car with the woman pulling around to the back of the building. Alicia Truss, child care staff member was outside at the time. Ms. Ogg asked Ms. Truss who was in the car and she told her it was Child B's Aunt and that is how they figured out who the woman was that supposedly asked for Child A. There were children being sent upstairs to the infant/toddler room. One of the children sent from the other room was Child B and she left with her aunt. On 6/17/21, I interviewed Ms. Truss who was on vacation. I asked her if she could tell me what happened the da
