Center staff members did provide appropriate care and supervision on 2/02/2026. Teacher 1 observed Child A and Child B pulling down their pants and she immediately stopped them and talked to them about this not being appropriate behavior. Teacher 1 did notify both Child A and Child B's parents regarding the incident. I was able to review attendance records for children and staff. The preschool room was found to be at a 2:11 ratio at the time of the incident and were within ratio.

On 2/2/2026, Child A and Child B pulled their pants down while playing. The center staff were able to catch this inappropriate contact immediately however failed to notify the department within 24 hours by phone, fax or email, as required.

On 2/2/2026, Child A and Child B pulled their pants down while playing. The center staff were able to catch this inappropriate contact immediately however failed to submit a written report to the department within 72 hours, as required.

There is not a preponderance of evidence that Teacher 1 denied Child A of the required privacy in the restroom. Teacher 1 and Child A's accounts of the incident inconsistent. This appears to be an isolated incident. Teacher 1 was advised to wait until the restroom was empty in the future if anything needs to be put away in the closet.
