On 12/20/2022 upon assignment of the intake, I initiated the special investigation with an onsite inspection at the facility. I met with program director (PD), Mindy Dole. She is the educational coordinator at the school who provided me with Child A's child information card, attendance records for 12/15/2022, written statements from both involved child care staff members, and the written policy on "Head Count/Transition list procedures." I obtained contact information for the two child care staff members who had been placed on 3 administrative leave pending an investigation by the Saginaw Intermediate School District (SISD). An agency wide meeting to retrain staff is scheduled for 01/06/2023, upon return from winter break. Ms. Dole said on 12/15/2022 around 3pm Child A's class came in from the playground. She was in the front door area letting parents into the building to sign out and pick up their children when SISD staff member and family advocate, Linda Stacy opened the door to Child A with an unrelated adult bringing her inside from the playground where he had found Child A alone. Ms. Dole escorted Child A back to her class. It was hectic as parents were there. Ms. Dole was unable to say much to the child care staff member (CCSM), Kathy Katsiouras, due to all the people who were around. Ms. Dole had each of the involved CCSMs, Miss Kathy, and Diamond Brazil, complete written, signed statements giving their account of what might have happened. The district sent out a letter to parents the following day on 12/16/2022 notifying them of the incident at the facility. According to the written statements from Miss Kathy and Miss Diamond, two children had been picked up early and released to their caregivers while outside on the playground that day. The attendance record for 12/15/2022 verifies that there were 13 children in attendance that day. Thirteen children went outside, two were picked up, and Miss Kathy and Miss Diamond were sure that they had counted 11 upon returning to the classroom. I called Kathy Katsiouras, CCSM, and interviewed her by telephone. She verified her written statement and that she has been employed in her capacity as a CCSM for five years. Nothing like this had ever happened to her before and she was sick over it. She was crying and emotional throughout the conversation stating several times that she swears she and Miss Diamond counted outside and again inside. She blames herself for what happened. Child A was not herself on the day of the incident, that Child A had to be prompted to answer when she was called on for attendance. Child A did not want to stay at school that morning which was also unusual for her. Child A was not even outside for five minutes. She did not even have her coat off before Ms. Dole returned Child A to class. She is trained in transitions and counting. I asked if she notified Child A's mother when Child A was picked up that afternoon about Child A being left unsupervised outside. She did not; she was stressed out from Ms. Dole bringing Child A back at the end of the day and forgot. Miss Kathy acknowledged that it shall be reported to the child's parent directly within 24 hours of a child being lost or left unsupervised. I called and interviewed Diamond Brazil, CCSM, who also verified her written statement and reported being in her second year of employment as a CCSM. Nothing like this had ever happened to her and it has been stressing her out because she cannot figure out how Child A was left outside. She adamantly maintained that she and Miss Kathy counted the children, used their transition lists, recounted upon entry to the school and the classroom getting 11 children every time. Unprompted she shared that Child A had been off on that day. Child A is typically 4 happy and active, but that day she was very clingy and had to be prompted to do things like answer when her name was called at attendance. On 2/4/2022 I conducted an onsite visit at Jerome Head Start Program Director Mindy Dole was spoken with regarding the incident. She provided a copy of the incident report and copies of the child information cards for Child A's classroom. She said on the day of the alleged incident, she noticed Child A in the hallways without a mask on and assisted him. Child A's mother then showed Ms. Dole a small bruise on Child A's chin. The child's teachers Child Care Staff Member 1 (CCSM1) and Child Care Staff Member 2 (CCSM2) were spoken with, and they reported Child A had bumped his chin on a door handle when he was coming in from a fire drill that had occurred that day. He was comforted by staff and seemed fine. There was no mark initially, but the bruise could have developed under his mask as the day progressed. The staff were 2 directed to fill out an incident report. Child A's mother was upset that she was not called but was told there was no mark and Child A seemed fine. The next week, Child A's mother said she did not think the bruise came from the door. Child A's mother requested Child A be moved from the class because she was uncomfortable with the staff in Child A's classroom. Child A was removed from the classroom and placed in a different room. There is a video camera in the entryway where Child A bumped into the door. The footage is in the process of being retrieved. Ms. Dole is unsure if the camera is operational. There have been no prior issues with Child A who has attended since . CCSM2 has worked for the licensee for more than fcivuer ryeenatlry*an as no previous issues or concerns noted in his past. He has only been in the classroom for a couple of weeks. CCSM2 was spoken with. He has worked for the " for a number of years but is new to the Jerome site since eis ware of the incident involving Child A. Child A was coming in from the fire drill. He was just ahead of Child A walking backwards so he could see the children in his class walking in a straight line. CCSM2 stepped to the side and as Child A continued walking, he was looking up at the children going upstairs and bumped his chin on the push bar of the entryway door. Child A started to cry. He stopped to comfort Child A and after a minute, Child A was fine. He checked on Child A throughout the day but saw nothing that concerned him and saw no differences in Child A's behavior. CCSM2 liked Child A and could not think of any other negative issues or incidents with him. At the end of the day Child A's mother was upset that she was not called. The conversation with Child A's mother got heated and he walked away. He knows Child A bumped his chin because he saw it happen. CCSM1 was spoken with. She confirmed that Child A bumped in to a door the day of the fire drill. CCSM2 was leading the children inside. The alarm was still going and the entryway was loud and chaotic. She heard Child A cry out loudly and looked to the front of the line and saw CCSM2 going over to Child A to comfort him. CCSM2 reported to her that Child A bumped into the push bar on the entryway door. She guessed that Child A was looking up at the kids going up the stairs and bumped the door handle with his chin. No mark was noticed throughout the day, but the kids wear masks so she would have only seen his face at limited times. Child A's mother came in that afternoon upset and pointed out the small mark on Child A's chin. Child A never complained or indicated he was uncomfortable. Child A's mother did not appear to like CCSM2 from the start. His conversation with Child A's mother did not go well so the decision was made to switch Child A's classroom. CCSM2 is great with kids, and he is a great male role model for the children in her classroom. He is gentle and caring with the children and would never hit, kick, or play roughly with the children.
