    R 400.1925 · R 400.1925 Comprehensive background check; fingerprinting. (1) Pursuant to section 5n of the act, MCL 722.115n, prior to an individual having any unsupervised contact with children, the department shall determine the individual's eligibility to be any of the following: (b) An adult member of the household.

    On 7/22/2021, I completed an unannounced on-site inspection at Tammy Straight Chavarria's group child care home. Also, present was child care consultant Mary Smith. I met with and interviewed licensee Tammy Straight Chavarria. Ms. Straight-Chavarria admitted she was out of ratio of personnel to children and should have been at a 1:6 ratio, however she did not have additional personnel present with her, as required by the rules, and was subsequently out of compliance at the time of the onsite inspection. ANALYSIS: Ms. Straight-Chavarria on 7/22/2021 was not in ratio, she had 12 children present and she was the sole caregiver. Ms. Straight-Chavarria called personnel Ms. Linda who subsequently came to the home within 10 minutes and placed the group child care home within ratio. A follow up unannounced on-site inspection was conducted on 08/10/2021 and there was a 2:6 ratio observed. CONCLUSION: I VIOLATION ESTABLISHED
