722.115n(3) Application for or renewal of license to operate child care center, group child care home, or family child care home; household member or child care staff member; criminal history check; requirements; duties of department. Each individual listed in subsection (1) shall give written consent at the time of the license application and before a group child care home or family child care home allows an individual to be a member of the household, or before becoming a child care staff member to allow the department of state police to conduct the criminal history check required under subsection (1). The department shall require the individual to submit his or her fingerprints to the department of state police and the Federal Bureau of Investigation for the criminal history check as required in subsection (1).

R 400.1911 · R 400.1911 Care; supervision; children. (1) A licensee shall ensure appropriate care and supervision of children at all times.

On 6/27/22, I interviewed the referral source. They stated that there were no specifics of children being inappropriate with anyone at the child care. It was not a "predatory situation" at the child care home. Ms. Lewis just cares for children that have been in some situations. The referral source did not elaborate about any situations at the child care home where the children were not supervised appropriately. On 6/28/22, I went to the child care home and interviewed Ms. Lewis. Ms. Lewis was not caring for any children and was expecting a family to arrive in a couple of hours. She is not aware of any children that she is caring for currently that have been inappropriate with any children. She has no idea what the allegations are referring to. No one has made Ms. Lewis aware of a situation that occurred at her home. She has cameras in the rooms and is very cautious. She can review the camera footage if anyone were to have a concern. Ms. Lewis has reviewed it for children's disputes to find out what exactly happened. She makes sure that she is very good about making sure the children are supervised in her home. Ms. Lewis currently will at the most have five children. She has two child care families and her grandson that will visit occasionally. She provided me the child information cards. I interviewed Child A's Mother. Her son visits the home maybe once or twice a month. She has never had any concerns regarding supervision while he is at her home. Child A's Mother stated, "Ms. Pat don't play like that". She is strict and makes sure the children are supervised and are appropriate in their behavior. Ms. Lewis makes sure any adults that are in her home act appropriately also. I interviewed Child B's Mother. She has been using Ms. Lewis for children care for a couple of months now. Child B (male, 11 months) attends three to four times a week. She has not had any concerns and feels that she could discuss concerns with her. Child B's Mother stated that Ms. Lewis is very good and makes sure the other children are gentle with Child B. I interviewed Child C, Child D, and Child E's Mother. She stated that the children have been attending Ms. Lewis's child care home for a couple of years now. She has no concerns and she feels that she could discuss them with Ms. Lewis if there were any concerns. 3
