    R 400.8125 · .............. R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 9/01/22, I conducted an on-site inspection. I interviewed Nicole Sootsman, program director. Ms. Sootsman explained that the preschool room was outside playing when the incident occurred. The child care staff, Tricia Kocinski and Martha Brown were having the 11 four-year-old children line up to go inside for lunch. At this time a child began vomiting. Ms. Brown assisted the ill child inside to take care of them. Ms. Koscinski brought the rest of the children inside the classroom. Ms. Brown continued to assist the ill child and contact their parents. Ms. Kosinski had the rest of the children sit down so they could start transitioning to lunch. About five minutes or so went by and Clarissa Gielda, child care staff member, brought Child A to the room. The staff did not realize that Child A was missing from the group. I interviewed Ms. Brown. Ms. Brown and Ms. Kocinski were outside with the four- year-old children. They lined the children up to go inside and one child started vomiting. Ms. Brown took that child inside to care for them. Ms. Kocinski remained outside and finished getting all the children ready to go inside. Ms. Kocinski brought the children inside. Ms. Brown finished taking care of the ill child while Ms. Kocinski was working on having the children transition to lunch. This is when Clarissa Gielda brought Child A into the classroom. She found him outside in the outdoor play area. Neither she nor Ms. Kocinski realized he was missing from the group. I then interviewed Ms. Gielda. Ms. Gielda was taking out trash and she was near the smaller outdoor play area. She heard something and looked at the other outdoor play area and saw Child A. Ms. Gielda did not see anyone else with child A. She then took Child A inside to the four-year-old classroom. Neither child care staff realized he was missing from the group. On 9/26/22, I interviewed Ms. Kocinski. Ms. Kocinski was having the children line up to go inside to the building. She remembers having a hold of Child A's hand at one point during this time to line the children up to go inside. Another child started vomiting and Ms. Brown took the child inside take care of them and to contact their parent. Ms. Kocinski thought all the children were lined up to go inside. She took the children inside to get ready for lunch. She did not realize Child A was missing until Ms. Gielda brought him to the classroom. She thought he may have been outside unsupervised for five to ten minutes. On 9/23/22, I interviewed Child A's Mother. She was informed of Child A being unsupervised right away after the incident occurred. This was Child A's first or second day in the preschool classroom as he just transitioned. He is very quiet and likes to do his own thing. He has received great care at the center. Ms. Sootsman informed Child A's Mother that she had a meeting and the procedures they were putting in place to ensure that this does not occur again. Child A's Mother felt comfortable with Ms. Sootsman's plan and has no other concerns. 3
