On 10/17/2025, Child A was walking in the hallway with Lead Teacher 1 when Child A left the line unnoticed and went into a bathroom. Child A was unsupervised in the bathroom. Witness 1 located Child A in the hallway unsupervised.

There are no findings that Claytor SISD Saginaw County violated prohibited discipline of hitting with children in care. Child Care Staff Members were present during alleged incident and did not witness physical discipline being used. Interviews were conducted with licensee designee, education coordinator, education manager, and child care staff member. Attempts were made to interview the father of the child in the alleged incident but were unsuccessful. Attempts were also made to interview parents of other children in care but were also unsuccessful.

Child care staff members did provide appropriate care and supervision of Child A. Child care staff members report when Child A arrived on 09/22/2022, he was not acting normal and went to sleep during lunch. When Child A woke up from his nap his lip was swollen, and Ms. Vaughn-Jones provided comfort to him, and they contacted Child A's mother. Ms. Herbert lead that when Child A arrived from he was not acting normal. At lunch which was 15-20 minutes after he arrived at the center, she noticed a small blister on his lip, and it appeared he had bit his lip. Both child care staff members denied Child A had any accidents or injuries at the center. Child care staff members contacted Child A's mother immediately when they noticed his liD was swollen. Child A is Per ChildA's mother she believes the injury happened at as he appeared to be scared and afraid when she took him back to the elementary school on 09/22/2022 and would not let his grandmother set him down. When Child A was with child care staff members at the center, Child A's mother reports he was happy and ran to the child care staff members with no fear.

On 05/05/2021, a telephone call was made to the licensee designee Shevonn Betts. Ms. Betts was not aware that Child A (female, age 4) tested positive for Covid-19. Ms. Betts will check with other caregivers and staff and call back. On 05/05/2021, I received a telephone call from Ms. Betts who reported there is a room closed at Claytor however it is not due to Covid-19. The center is closed at this time and she will investigate this on 05/06/2021 as she was not made aware of a positive Covid-19 test result at the center. On 05/06/2021, an unannounced on-site inspection was completed at Claytor SISD Saginaw County Head Start. I met with lead caregiver Malerie Hetrick in Room 5 and an interview was conducted. On Monday 05/03/2021, Child A was not present. Staff sent Child A's mother a message at 9:12 a.m. via Dojo (an application that is used to communicate with parents) and the message was checking on Child A and the reason for her absence. At 10:44 a.m. Child A's mother wrote back via Dojo and stated Child A was not feeling good and that she will return the next day. During nap time on 05/03/2021 staff called Child A's mother for a transitional conference that was already scheduled. At that time Child A's mother had just sent staff a message via Dojo that stated, "Hello was writing to let the school know that tested positive for Covid". During the transitional conference staff asked Child A's mother how Child A was feeling and that is when staff were informed that the entire family except for mother had tested positive for Covid-19 and she had just received the rapid test results. Child A's mother reported that the children were asymptomatic however her husband had a headache and fever. Ms. Hetrick then reported the positive Covid-19 to the program director Kristin Clifford immediately following the transitional conference which was at approximately 2:00 p.m. on 05/03/2021. Ms. Hetrick reports that the center sent parents notification of a positive Covid-19 result on 05/04/2021 in the afternoon via Dojo. Ms. Hetrick reports that staff and children wear masks the entire time they are in the center. The only time children do not wear masks is when they are napping. The last time Child A was at the center was on Thursday 4/29/2021 and she tested positive for Covid-19 on 05/03/2021. Staff were informed by the program director, Ms. Clifford that no one had to quarantine as Child A had not been at the center in over 48 hours. I then met with and interviewed caregiver Jessica Willingham in Room 5. On Monday 05/03/2021, Child A was not present. Staff sent Child A's mother a message at 9:12 a.m. via Dojo (an application that is used to communicate with parents) and the message was checking on Child A and the reason for her absence. At 10:44 a.m. Child A's mother wrote back via Dojo and stated Child A was not feeling good and that she will return the next day.
