The center failed to provide appropriate care and supervision to children when Teacher 1 and Teacher 2 left Child A unattended sleeping in a classroom for eight minutes on 4/13/2026.

On 1/28/2026, Teacher 2 did not use positive methods of discipline when holding Child A's hands and arms at the table as this form of discipline does not encourage self-control, self-direction, self- esteem and cooperation.

Teacher 3 told several teachers and the licensee designee/program administrator she thinks Teacher 2 slapped her own child's hand. A review of video surveillance in the room did not reveal Teacher 2 slapping Child B. Teacher 2 is no longer employed at the center.

On 2/18/2026, during an unannounced onsite inspection, I observed two infants sleeping in swings which are not approved for sleeping. Lead teacher 1 and Teacher 1 reported they did not realize the infants had fallen asleep until I brought it to their attention.

Based on the investigative findings, there is insufficient evidence to show that there was improper care or supervision of Child A. Child A was in the self-contained restroom for a few minutes alone, but the door was open and child care staff checked on her. Child A has a history of scratching herself and no one that was spoken with believes that the marks on Child A’s face came from a staff member.
