On 07/28/2021 licensee designee Steffanie Bowles emailed a written report about this accident. The report documented that Child D was standing on two stacked milk crates. Child D jumped off to hit a ball that was on top of a sunshade. Child D lost her balance and fell on her outstretched left arm. Child D's arm was bent in an unnatural position. Staff applied ice packs to Child D's arm and wrapped the ice packs with gauze to stabilize the arm. On 08/03/2021 I spoke with Child D's Mother. Child D's Mother knew that Child D broke her arm when she jumped off two stacked milk crates and fell. She stated that Child D is doing great. Child D's Mother had no concerns about what had happened or how the staff handled things. She stated the staff were prompt in responding. On 08/03/2021 I completed an on-site inspection to the center. I observed the play area where Child D was injured. There were a few milk crates in the play area. Each milk crate is approximately 11 inches high. At the time of my inspection the milk crates were not stacked, and the children were not playing with them. The play area ground cover had some wood chips on it, but the area where Child D fell was hard packed dirt. I interviewed licensee designee Steffanie Bowles. She stated at the time of Child D's accident, she was inside the building and did not see what happened. Ms. Bowles stated staff members Cullen Conway and Olivia Beelby were outside with some of the preschool children, including Child D. Mr. Conway brought Child D inside after the accident, and she and Mr. Conway applied ice and stabilized Child D's arm. I interviewed program director Jamie Solomon. She stated she was inside the building and did not see Child D fall outside. Ms. Solomon stated Mr. Conway brought Child D inside, and he and Ms. Bowles provided first aid to Child D. Ms. Solomon stated she called Child D's Mother and Father and initially had some difficulty reaching them. She left messages for them, and they called back about 20 minutes later. I interviewed staff member Cullen Conway about the accident. He stated that he and Olivia Beelby were outside with five or six preschool children. Child D and another child were playing a game where they stand on milk crates, throw a ball onto a sunshade, and jump off the milk crates to catch the ball as it rolls off the sunshade. Mr. Conway stated they had a "rule" that the children could only stack milk crates two high. He stated Child D had stood on and jumped off two stacked milk crates before this accident and had never fallen off them. He stated Child D "was really good at jumping". Mr. Conway did not see Child D fall but heard Child D cry and ran over to Child D and saw that her arm looked broken. He took Child D inside, and Steffanie Bowles helped give first aid to Child D. Mr. Conway stated after this accident happened, they took down the sunshade. 3 I interviewed staff member Olivia Beelby. She stated she and Cullen Conway were outside that afternoon with a small group of six of the older children. Several of the children, including Child D, were playing a game throwing the ball to each other. Ms. Beelby stated the children were also playing on the milk crates. She stated the children do a "stair step" using one or two of the milk crates and "typically" stand on only two milk crates at a time. Ms. Beelby did not see Child D fall but heard the milk crates coming down and crying; when she looked, she saw Child D on the ground. Ms. Beelby then saw Child D stand up, and Child D was clutching her arm. Ms. Beelby stated Cullen Conway went running over to Child D, and he took Child D inside.
