On 02/06/2023, I received a telephone message from Rashidah Hammond, education specialist and licensee designee. Ms. Hammond indicated that there was an incident with a child care staff member and a child bumped his head. On 02/07/2023, I conducted a telephone interview with Ms. Hammond. She received a telephone call on 02/06/2023 from Child A's Mother indicating that Child A said a teacher pushed him and he bumped his head. Child A's Mother was upset that she was not notified of the incident. Ms. Hammond informed Child A's Mother that she 2 would follow up with the child care staff members. Child A's Mother requested that Child A not be placed back in the same classroom. Ms. Hammond then contacted child care staff member, Colleen Onnie, to ask what happened with Child A. Ms. Onnie reported that at the end of the day as the children were lining up to leave, she had Child A by the hand, and he let his "feet fall out" from under him and fell. Another child care staff member, Sara Krasnicki, was in the room and observed the incident. I asked Ms. Hammond to submit an incident report and informed her that a special investigation would be opened. On 02/07/2023, I conducted a telephone interview with Child A's Mother. She stated that on 02/06/2023, Child A's Father picked up Child A from school and one of the "teachers", she did not know which one, told Child A's Father that Child A had been "hyper". Child care staff members did not mention that Child A had fallen. Child A's Father asked Child A what happened at school and Child A reported that a teacher had pushed him, and he hit his head. Child A reported that his head was hurting. When Child A got home, he told Child A's Mother the same thing, that a teacher had pushed him, and he fell. Child A demonstrated how his arm had been picked up behind his head and how he was pushed to the floor. Child A's Mother then sent a message on the app asking what happened. Ms. Onnie responded on the app that Child A had fallen and acknowledged that she had forgotten to tell her about it. Ms. Onnie then called Child A's Mother to discuss the fall. Child A's Mother informed Ms. Onnie that Child A said she grabbed him by the arm, pushed him and he hit his head when he fell. Ms. Onnie informed Child A's Mother that she did not like where the conversation was going, and she was going to notify supervision of Child A's Mother's concerns. Child A's Mother was upset that she was not notified of the incident. Child A's Mother does not want Child A returning to the same classroom and has requested he be put in another classroom. Child A had previously said that the teachers were being mean to him, and he did not want to go to school. She was waiting to hear where Child A would be moved to. During the telephone interview with Child A's Mother, she indicated that Child A's Father was present and could also be interviewed. I then interviewed Child A's Father on the telephone. Child A's Father stated that he picked up Child A on 02/06/2023 and Ms. Onnie was "not herself'. She did not say anything about Child A falling and she only said that Child A was not listening that day. When they got to the car, Child A said that the teacher threw him to the floor. Child A then demonstrated falling to the floor. Child A's Father let Child A's Mother know what he said. When he picked up Child A, Child A looked fine and did not appear to be in distress or like he had been crying. He denied observing any injury but thought Child A's head "may have been a little red". 3 Child A's Father normally picks up Child A from school and this was only the second time the child care staff members had said anything about Child A's behavior in the classroom. Prior to this incident, Child A's Father did not have any concerns about the care provided to Child A at the center. On 02/08/2023, I conducted an unannounced onsite inspection at Dort Head Start.
