On 10/27/2020, I received a telephone call from Katherine Landini, licensee designee and program director, regarding the serious injury of a child. Ms. Landini indicated that Child A's Mother called on 10/27/2020 to report that she had taken Child A to the hospital the evening of 10/26/2020. Child A was diagnosed with a fractured right femur. Ms. Landini stated that Child A had been fussy on 10/26/2020 and went home early. She indicated that the only incident that happened on 10/26/2020 was while Child A was in the gross motor room with Child B, he tried to stand by pulling himself up at the bottom of the slide. Child A then plopped down on his butt after trying to stand. I asked Ms. Landini to submit a written incident report to the department. On 10/27/2020, I received the written incident report from Ms. Landini. The report indicated that on 10/27/2020, Child A's Mother called the center to report that Child A had been fussy after picking him up on 10/26/2020. Child A would not put weight on his right leg, so she took him to the emergency room where he was diagnosed with a fractured femur. The incident report indicated that on 10/26/2020, Child A's Mother was called to pick up Child A from care after he was "fussy and seemed uncomfortable". While on the phone, Child A's Mother reported that Child A had been teething, but that he had a good weekend. Child A's Mother was informed at pick up that when Child A was in the large motor room earlier that morning, he pulled himself up on the side of the slide. Another child went down the slide and Child A "sat down from a standing position". Child A went home around 10:30a.m. on 10/27/2020 and Child A's Mother was called at 1:00p.m. to see how Child A was doing. Child A's Mother reported that Child A was fine, but was fussy. On 10/27/2020, I conducted a telephone interview with Child A's Mother. Child A's Mother stated that on 10/26/2020, she dropped Child A off at the center around 7:00a.m. She stated that Child A was fine and "normal" when she dropped him off. She stated it was sometime after 9:00a.m. when she received a telephone call from the center stating that Child A was fussy and crying. I asked Child A's Mother if she knew who called her and she stated she thought it was "Ms. Lana". I asked if she meant Ms. Lynn or Ms. Lawton and she stated she was not sure. Child A's Mother stated that she arrived around 10:00a.m. to take Child A home from the center. 3 Child A's Mother stated that when she arrived, she was informed that Child A had been fine and was playing "like normal" until he pulled himself up by the side of the slide. When another child slid down, Child A fell down. Child A's Mother stated she was told that Child A continued to cry for around an hour and a half after falling which is when she was contacted. Child A's Mother stated she took him home and during the day he played, but was fussy. She stated later that evening she noticed that he would not put any weight on his leg and when she tried to get him to stand or bear weight, he would not. Child A's Mother took Child A to the emergency room where he was diagnosed with a fractured femur. Child A's Mother stated the doctor said it was possible for a child to receive a broken femur from falling, but usually more force was needed for that kind of break. Child A's Mother stated that an entire body scan, x-rays, blood work and urine test were taken, and all results were negative. She further indicated that Child A did not have any marks, bruises or injuries when examined at the hospital. Child A's Mother has been taking Child A to the center since this past summer and "for the most part" has been satisfied with the care he receives. She indicated that there have been a couple of issues with food being on his face at pick up and one time he had a soiled diaper, but she has been satisfied with the care he receives. She stated that the child care staff members are "really nice" and it seems like "a great atmosphere". Chi
