On 09/19/2023, I spoke to Child A’s Mother. She stated that on 9/12/2023, Child A’s Grandfather went to the school to pick Child A up. He was told she was not there and that she was released to a child care worker for a different child care center. In the past Child A would get picked up by another child care center for afterschool care. The family was not using this child care center for afterschool care this year. Child A’s Mother stated that she told CCSM 1 on several occasions that Child A was not attending the child care center for after school care this year. She added Child A’s Grandfather to the child information card at the home visit they had recently had and said that he would be picking Child A up. When Child A’s Grandfather was told she was not at the school, he drove to the child care center but the bus driver, who works at the child care center, had not arrived onsite yet. She was later called by the CCSMs at Romulus Head Start- Cory Elementary and was told Child A was being brought back to the school. On 09/20/2023, I completed an unannounced inspection at Romulus Head Start - Cory Elementary. I met with Program Director, Megan Anderson and CCSM 2. Ms. Anderson stated that on 9/12/2023 CCSM 1 released Child A to an unauthorized individual. The individual is a bus driver and child care staff member from a child care center that Child A attended last year. The school is using tablets this year to sign children in and out. On the tablets the staff can see who is allowed to pick up each child. She was told by CCSM 1 that CCSM 1 and Child A walked over to the door, CCSM 1 handed the tablet to the bus driver, and he signed the child out. A few minutes later, Child A’s Grandfather came to the school to pick Child A up and CCSM 1 realized she released the child to an unauthorized individual. She called CCSM 2, who is a family service worker, down to the classroom. Ms. Anderson happened to be walking in the hallway at this time and CCSM 1 and CCSM 2 informed her what happened. CCSM 2 called Child A’s Mother to explain what had happened. They were able to locate Child A and the bus driver brought Child A back to school where she was then released to the Child A's Grandfather. When Ms. Anderson spoke to the bus driver, he said he knew he was there to pick up a child but did not know what classroom the child was in this year. He went to a classroom he went to last year and was immediately recognized. He said he did not ask for a specific child but that the teacher recognized him and brought Child A over to him. I spoke to CCSM 2 who shared the same thing as Ms. Anderson. She said that she called Child A’s Mother as well as the childcare center that the bus driver worked for to figure out where Child A was. She was finally able to get ahold of the child care 3 center and they informed her that the bus driver was taking Child A back to the school. Once Child A arrived onsite, she was released to Child A’s Grandfather. On 09/22/2023, I spoke to the bus driver. He stated that he arrived at the school, knew he was there to put up one child this year, last year it was two children. He stated he is not the regular driver but helps out when needed. He was there to pick up Child B, who is a 4-year-old female, but could not remember what she looked like and did not know what class she was in. He stated he walked to one of the classes that he went to last year and CCSM 1 immediately recognized him and handed him a tablet to sign out Child A. He said he thought Child A was Child B so he signed her out. He said that CCSM 1 never said the child’s name. When asked if he told CCSM 1 that he was there to pick up Child B he said he could not remember if he said Child B’s name, but he thinks he did. When asked if CCSM 1 used Child A’s name he replied “maybe, maybe not, can’t remember”. On 10/03/2023, I spoke to CCSM 1. She said that last year there was an assistance in the classroom and Child A was allowed to be released to the bus driver fo
